  • Medication packaging label: A clear photo of the bottle or blister pack label—with the drug name, strength, and batch number—also works. This confirms you have that exact medication.

  • Detailed description if you lack documents: If you don’t have any of the above, describe who prescribed the medication, when it was issued, how long you’ve taken it, and any side effects experienced. The more detail you provide, the faster our doctor can assess your case.
