1. Qué es Takipril hiperbárica y para qué se utiliza

Takipril hiperbárica 20 mg/ml es un tipo de medicamento llamado anestésico local, que pertenece a la categoría de las amidas. Takipril hiperbárica es una solución inyectable que se utiliza para anestesiar (dormir) partes específicas del cuerpo y evitar el dolor durante la cirugía en adultos.

Takipril hiperbárica se inyecta en la parte más baja de su columna vertebral. Esto frena rápidamente el dolor de cintura para abajo por un periodo limitado de tiempo (cirugías de corta duración).

2. Qué necesita saber antes de empezar a usar Takipril hiperbárica

No se le debe administrarTakipril hiperbárica

  • si es alérgico (hipersensible) al clorhidrato de prilocaína, a otros anestésicos locales de tipo amida o a cualquiera de los demás componentes de este medicamento (incluidos en la sección 6)
  • si tiene problemas graves de la conducción cardíaca
  • si sufre de anemia grave
  • si padece una insuficiencia cardíaca descompensada
  • si tiene shock cardiogénico e hipovolémico
  • si sufre de metahemoglobinemia congénita o adquirida.
  • si tiene contraindicaciones específicas o generales para la técnica de la anestesia subaracnoidea

No se le puede administrar Takipril hiperbárica en los vasos sanguíneos.

Takipril hiperbárica no se puede usar en niños menores de 6 meses.

Advertencias y precauciones

Si se identifica con cualquiera de las siguientes situaciones, debe comentarlo con su médico antesde que se le administre este medicamento.

  • si ha sufrido alguna vez una reacción adversa a un anestésico en el pasado
  • si tiene una infección cutánea en el lugar propuesto para la inyección o en sus proximidades
  • si sufre cualquiera de los siguientes trastornos:
  • enfermedades del sistema nervioso central como meningitis, polio o problemas de la médula espinal debidos a una anemia
  • dolor de cabeza grave
  • tumores cerebrales, raquídeos o de otra localización
  • tuberculosis raquídea
  • traumatismo reciente en la columna vertebral
  • presión arterial muy baja o bajo volumen de sangre
  • problemas de la coagulación sanguínea
  • porfiria aguda
  • líquido en los pulmones
  • septicemia (intoxicación de la sangre)
  • si padece una enfermedad cardíaca (por ejemplo un bloqueo cardíaco total o parcial, descompensación cardíaca o arritmia)
  • si tiene problemas hepáticos o renales
  • si sufre de trastornos neurológicos tales como esclerosis múltiple, hemiplejia, paraplejia o trastornos musculares.
  • si se encuentra en un estado de salud general bajo

La anestesia espinal debe solo ser administrada por un médico con los conocimientos y experiencia necesarios. El médico es responsable de tomar las medidas necesarias para evitar la infeccción en el vaso sanguineo y saber reconocer y tratar los efectos adversos.

Niños y adolescentes

Takipril hiperbárica no está recomendado para el uso en niños y adolescentes. La eficacia y seguridad de Takipril hiperbárica en población pediátrica no se ha establecido. No hay datos disponibles.

El uso de Takipril en niños menores de 6 meses está contraindicado debido al mayor riesgo de

desarrollar metahemoglobinemia.

Otros medicamentos y Takipril

Comunique a su médico si está utilizando, ha utilizado recientemente o podría tener que utilizar cualquier otro medicamento incluso los adquiridos sin receta. En especial si está tomando algún medicamento para el ritmo cardíaco irregular (antiarrítmicos de clase III) y el alivio del dolor.

Embarazo y lactancia

Si está embarazada o en período de lactancia, o cree que podría estar embarazada o tiene intención de quedarse embarazada,su médico decidirá si le administrará la inyección de Takipril hiperbárica. La prilocaína no debe ser administrada como anestesia local o regional durante el parto.

No se sabe si la prilocaína se excreta con la leche materna. Si es necesario administrarla durante la lactancia, ésta puede reanudarse unas 24 horas después del tratamiento.

Conducción y uso de máquinas

No conduzca ni maneje herramientas o máquinas, ya que Takipril hiperbárica puede interferir temporalmente con su coordinación muscular y sus reacciones.

Takipril hiperbárica contiene sodio

Este medicamento contiene menos de 1 mmol de sodio (23 mg) por dosis (dosis máxima igual a 4 ml de Takipril hiperbárica), por lo que se considera esencialmente “exento de sodio”.

3. Cómo usar Takipril hiperbárica

Este medicamento se lo administrará su médico, quien decidirá cuál es la dosis correcta para usted. La dosis usual en adultos es de 40 – 60 mg de prilocaína hidrocloruro (2-3 ml de Takipril hiperbárica); la dosis máxima es de 80 mg de prilocaína hidrocloruro (4 ml de Takipril hiperbárica).

Su médico le administrará Takipril hiperbárica en la parte más baja de su columna vertebral mientras usted se encuentra sentado o tumbado.

Uso en niños y adolescentes

Takipril hiperbárica no está recomendado para el uso en niños y adolescentes. La seguridad y eficacia de Takipril hiperbárica en población pediátrica no ha sido establecida. El uso de Takipril hiperbárica en niños menores de 6 meses está contrainidicado debido al mayor riesgo de desarrollar metahemoglobinemia.

En los pacientes con afectación del estado general y con enfermedades concomitantes establecidas (p. ej., oclusión vascular, arteriosclerosis, polineuropatía diabética) está indicada una dosis reducida.

En caso de un hígado o riñones comprometidos, se recomienda un rango menor de dosis.

Takipril hiperbárica se inyecta por vía intratecal.

Debe disponerse de forma inmediata del equipo, los fármacos y el personal cualificado necesarios para afrontar una emergencia. En raros casos se han comunicado reacciones graves tras el uso de anestésicos locales, incluso en ausencia de hipersensibilidad individual en los antecedentes clínicos del paciente.

Si se le administra más Takipril hiperbárica del que debe

El médico que le administra Takipril hiperbárica tiene experiencia en el uso de anestésicos locales por vía intratecal, por lo tanto es improbable que se le administre una sobredosis. Sin embargo, si se le inyecta accidentalmente la dosis en la sangre, pueden aparecer problemas con su vista u oído por un periodo corto de tiempo, contracción de los músculos, temblores, convulsiones y pérdida de conciencia. Cuando se le administre Takipril hiperbárica y en caso que una sobredosis tenga lugar, un equipo estará disponible para asistirle.

4. Posibles efectos adversos

Al igual que todos los medicamentos, Takipril hiperbárica puede producir efectos adversos, aunque no todas las personas los sufran.

Como ocurre con todos los anestésicos locales, pueden producirse una caída de la presión arterial y una ralentización de la frecuencia cardíaca.

Puede notar ganas de vomitar y tener la presión arterial baja o un ritmo cardíaco lento. Otros posibles efectos son dolor de cabeza tras la cirugía, vómitos y dificultad para orinar.

Estos son los posibles efectos adversos:

Muy frecuentes: pueden afectar a más de 1 de cada 10 personas

Disminución de la presión arterial, náuseas

Frecuentes: pueden afectar hasta 1 de cada 10 personas

Parestesia, mareos, vómitos

Poco frecuentes: pueden afectar hasta 1 de cada 100 personas

Convulsiones, parestesia circumoral, pérdida de conciencia, temblor, sensación de adormecimiento que afecte a la lengua, problemas del habla, problemas de oído, tinnitus, problemas visuales, dolor de espalda, debilidad muscular temporal, latido lento del corazón. Presión arterial elevada.

Raras: pueden afectar hasta 1 de cada 1000 personas

Metahemoglobinemia, cianosis. Choque anafiláctico, reacción anafiláctica, picor. Aracnoiditis, neuropatía, lesiones en los nervios periféricos.

Diplopía. Paro cardíaco, latidos irregulares. Depresión respiratoria.

Es improbable que Takipril hiperbárica solución inyectable cause efectos adversos graves a menos que se inyecte accidentalmente de forma errónea o se utilice conjuntamente con otros anestésicos locales. Si esto ocurre, pueden producirse adormecimiento de la lengua, sensación de desfallecimiento, mareo, temblor y ataques convulsivos. En casos extremadamente raros, la prilocaína se ha asociado con ataque cardíaco, dificultades respiratorias, pérdida de la sensibilidad en la parte inferior del cuerpo y reacciones alérgicas, que pueden causar erupciones cutáneas, hinchazón o cifras de presión arterial muy bajas.

Una reacción adversa rara, pero grave, de la anestesia intratecal es el bloqueo espinal alto o completo, con la consiguiente depresión cardiovascular y respiratoria.

Si experimenta efectos adversos, consulte a su médico o farmacéutico o enfermero incluso si se trata de efectos adversos que no aparecen en este prospecto.

5. Conservación de Takipril hiperbárica

Mantener este medicamento fuera del alcance y de la vista de los niños.

No utilice Takipril hiperbárica después de la fecha de caducidad que aparece en la etiqueta de la ampolla y en la caja externa. La fecha de caducidad es el último día del mes que se indica.

No conservar Takipril hiperbárica a temperatura superior a 25°C. No refrigerar o congelar. Conservar en el embalaje original para protegerlo de la luz.

El medicamento debe usarse inmediatamente después de su primera apertura.

No utilice Takipril hiperbárica si observa que la solución no presenta un aspecto claro y carente de partículas.

Todo producto restante debe ser eliminado. Dado que su uso está limitado a hospitales, la eliminación de los residuos del medicamento la realiza directamente el hospital. De esta forma ayudará a proteger el medio ambiente.

6. Contenido del envase e información adicional

Composición de Takipril hiperbárica

  • El principio activo es Prilocaína hidrocloruro.

1 ml de solución inyectable contiene 20 mg de Prilocaína hidrocloruro (equivalentes a un 2%).

1 ampolla con 5 ml de solución contiene 100 mg de clorhidrato de prilocaína.

  • Los demás componentes son: glucosa anhidra o glucosa monohidrato, hidróxido de sodio 1N (para ajuste del pH) y agua para preparaciones inyectables.

Aspecto del producto y contenido del envase

Solución inyectable. Solución clara e incolora.

Takipril hiperbárica se presenta en ampollas de vidrio claro incoloro de tipo I.
