1. Qué es NOXAP y para qué se utiliza

NOXAP es una mezcla de gases para su uso por inhalación.

NOXAP es un gas medicinal, comprimido, consistente en una mezcla de gases que contiene 200 ppm mol/mol de óxido nítrico.

¿Cuáles son sus usos?

NOXAP debe ser administrado exclusivamente por profesionales de la salud y es sólo para uso estrictamente en hospitales. NOXAP está indicado en los siguientes casos:

En los recién nacidos con una falta del oxígeno en la sangre (insuficiencia respiratoria hipoxémica) relacionada con diferentes causas.

NOXAP mejora la oxigenación y reduce la necesidad de circulación

En los pacientes pediátricos y adultos sometidos a cirugía cardíaca con alta presión arterial aguda en los pulmones(hipertensión pulmonar). NOXAP reduce la hipertensión pulmonar y el riesgo de insuficiencia del corazón derecho.

2. Qué necesita saber antes de empezar a usar NOXAP

No tome NOXAP en los siguientes casos:

  • Recién nacidos con dependencia comprobada al cortocircuito de sangre derecha-izquierda o que sufren un cortocircuito izquierda-derecha significativo.
  • Pacientes con deficiencia congénita o adquirida de metahemoglobina reductasa (MetHb reductasa) o de glucosa 6 fosfato deshidrogenasa (G6PD).

Tenga especial cuidado con NOXAP:

  • En los pacientes con disfunción del ventrículo izquierdo, NOXAP podría causar insuficiencia cardíaca y edema pulmonar.
  • En los pacientes con defectos cardíacos congénitos complejos.

Uso de otros medicamentos

Informe a su médico si está utilizando o ha utilizado recientemente otros medicamentos, incluso los adquiridos sin receta.

Oxígeno:En presencia de oxígeno, el óxido nítrico se oxida rápidamente, produciendo sustancias que son tóxicas para los pulmones. Para evitar esta situación, durante el tratamiento con NOXAP debe realizarse una monitorización continua.

Donantes de NO:Los medicamentos de uso cardiológico como nitroprusiato de sodio y nitroglicerina pueden tener un efecto aditivo al de NOXAP, aumentando el riesgo de presentar metahemoglobinemia.

Fármacos que aumentan las concentraciones de metahemoglobina: Cuando los fármacos como alquilnitratos, sulfamidas y prilocaína se administran junto con el óxido nítrico, hay un aumento del riesgo de presentar metahemoglobinemia.

Se han comunicado efectos sinérgicos con la administración de vasoconstrictores (almitrina, fenilefrina), prostaciclina e inhibidores de la fosfodiesterasa, sin aumentar las reacciones adversas.

El óxido nítrico inhalado se ha usado al mismo tiempo que tolazolina, dopamina, dobutamina, norepinefrina, esteroides y sulfactantes, sin que se hayan observado interacciones.

El médico decidirá si NOXAP puede utilizarse con otros medicamentos y vigilará atentamente su tratamiento.

Embarazo y lactancia

NOXAP no debe emplearse durante el embarazo a no ser que el estado clínico de la mujer requiera el tratamiento con NOXAP.

Pueden esperarse efectos dañinos ya que la metahemoglobina se considera perjudicial para el feto y el óxido nítrico ha demostrado un potencial genotóxico mediante la inducción de alteraciones estructurales en el ADN.

Se debe interrumpir la lactancia materna durante el tratamiento con NOXAP.

Conducción y uso de máquinas

No se han comunicado estudios sobre la capacidad para conducir y utilizar máquinas.

3. Cómo usar NOXAP

NOXAP debe ser administrado exclusivamente por profesionales de la salud y es sólo para uso estrictamente en hospitales.

NOXAP se administra por vía inhalatoria por medio de un sistema que administra en los pulmones la concentración prescrita de óxido nítrico, mediante la dilución de NOXAP en una mezcla de oxígeno y aire.

Su médico determinará la posología y la duración de su tratamiento con NOXAP.

Si toma más NOXAP del que debiera,el sistema de administración activará una señal de aviso. Entonces, su médico disminuirá o detendrá la administración de NOXAP y, posteriormente, determinará el tratamiento más adecuado que debe seguirse.

Si interrumpe el tratamiento con NOXAP

Su médico disminuirá gradualmente la dosificación de NOXAP hasta el final del tratamiento.

El tratamiento con NOXAP no debe interrumpirse abruptamente, sino reducirse lentamente a fin de permitir que los pulmones se adapten a la concentración normal de oxígeno en el aire.

4. Posibles efectos adversos

Al igual que todos los medicamentos, NOXAP puede tener efectos adversos, aunque no todas las personas los sufran.

Su médico observará y vigilará atentamente cualquier efecto adverso. No es probable que usted sufra estos efectos adversos.

Trastornos de la sangre y del sistema linfático

  • Un aumento de la concentración de óxido nítrico en la sangre puede acarrear el riesgo de presentar metahemoglobinemia. En este caso, la capacidad de transporte de oxígeno por la sangre puede disminuir. Si esto ocurre, su médico disminuirá inmediatamente la dosificación de óxido nítrico, para permitir que la sangre alcance su capacidad normal de transporte de oxígeno. En los pacientes pediátricos y en otros con niveles reducidos de la enzima metahemoglobina reductasa, este riesgo puede aumentar. La formación de metahemoglobina con un nivel sérico de > 5% con concentraciones de óxido nítrico inhalado de < 20 ppm es muy rara (< 1/10.000).
  • Tiempo de sangría:En un estudio preclínico se ha observado que el óxido nítrico inhalado aumenta el tiempo de sangría. Sin embargo, en estudios clínicos controlados que se han realizado, no se han encontrado diferencias significativas entre el grupo testigo y el grupo con tratamiento en lo que respecta a las complicaciones hemorrágicas.

Trastornos generales y alteraciones en el lugar de administración

  • Puede observarse una falta derespuestaal tratamiento en el 30 al 45% de los casos.
  • El aumento significativo de las concentraciones de NO2 a dosis terapéuticas bajas (< 20 ppm) de NO inhalado, con evidencias de toxicidad clínica por el NO2, es una complicación muy rara (< 1/10.000).
  • La interrupción abrupta del tratamiento con óxido nítrico inhalado, causa con mucha frecuencia (> 1/10) reacciones rápidas de rebote que presentan una intensificación de la vasoconstricción pulmonar e hipoxémica.

5. Conservación de NOXAP

Mantener fuera del alcance y de la vista de los niños.

No usar NOXAPdespués de la fecha de caducidad que aparece en la etiqueta del envase, después de “CAD”.

Deben seguirse todas las normativas relativas a la manipulación de envases a presión.

El almacenamiento es supervisado por los especialistas locales del hospital. Los envases deben almacenarse en habitaciones bien ventiladas o en zonas a cubierto ventiladas, donde estén protegidos de la lluvia y la luz solar directa.

Proteger los envases de los golpes, caídas, materiales oxidantes e inflamables, humedad, fuentes de calor o ignición.

Conservación en el departamento de farmacia

Los envases deben almacenarse en un lugar designado exclusivamente para el almacenamiento de gases medicinales, que esté bien ventilado, limpio y cerrado con llave. Este lugar debe albergar un lugar separado y especial para el almacenamiento de envases de óxido nítrico.
