1. Qué es CLORURO DE SODIO FRESENIUS KABI y para qué se utiliza

CLORURO DE SODIO FRESENIUS KABI es una solución inyectable, que pertenece al grupo de medicamentos denominados soluciones electrolíticas.

CLORURO DE SODIO FRESENIUS KABI está indicado en el tratamiento de la hiponatremia (bajo contenido de sodio en sangre) severa de diferentes causas

2. ANTES DE USAR CLORURO DE SODIO FRESENIUS KABI

No use CLORURO DE SODIO FRESENIUS KABI:

  • si es alérgico (hipersensible) al principio activo o a cualquiera de los demás componentes de Cloruro de sodio Fresenius Kabi.
  • si padece de edema (acumulación excesiva de líquido)
  • si padece de eclampsia (convulsiones no atribuidas a otra causa durante el embarazo)
  • si padece de hipernatremia (alto contenido de sodio en sangre)

Tenga especial cuidado con CLORURO DE SODIO FRESENIUS KABI:

El cloruro de sodio debe ser utilizado con extrema precaución en pacientes con fallo del corazón, insuficiencia renal grave (disminución importante de la función del riñón), cirrosis hepática (enfermedad crónica progresiva del hígado) y en los que reciben corticoides o corticotropina (medicamentos utilizados como antinflamatorios). También debe tenerse especial cuidado en pacientes ancianos y convalecientes de una operación.

El exceso de cloro en el organismo puede originar una pérdida de bicarbonato con la consiguiente disminución del pH. La administración intravenosa excesiva de cloruro de sodio puede también dar lugar a un bajo contenido de potasio en sangre.

La coexistencia de oxigenación insuficiente de la sangre y del bajo contenido de sodio en sangre, puede provocar lesiones cerebrales aún en ausencia de cualquier corrección del bajo contenido de sodio en sangre.

Se debe administrar con precaución en pacientes con la tensión sanguínea alta.

Si se administra continuamente en el mismo lugar de inyección puede producirse dolor, infección y flebitis (inflamación de la pared de una vena).

Consulte a su médico, incluso si cualquiera de las circunstancias anteriormente mencionadas le hubieran ocurrido alguna vez.

Uso de otros medicamentos:

Antes de mezclar con otros medicamentos se deben comprobar las tablas de compatibilidad, tener en cuenta el pH y controlar los iones.

La administración conjunta de cloruro de sodio y corticoides o corticotropina (medicamentos utilizados como antinflamatorios), puede dar lugar a una acumulación excesiva de líquido debido al sodio que contienen.

Se considera clínicamente importante la interacción del sodio con el litio (medicamento usado para el tratamiento de algunas enfermedades psiquiátricas). Un exceso de sodio puede impedir por defecto el establecimiento de niveles adecuados de litio y por el contrario el defecto de cloruro de sodio puede dar lugar a un incremento de los niveles de litio hasta valores tóxicos.

Tenga en cuenta que estas instrucciones pueden ser también de aplicación a medicamentos que se hayan utilizado antes o puedan utilizarse después.

Informe a su médico o farmacéutico si está utilizando o ha utilizado recientemente cualquier otro medicamento, incluso los adquiridos sin receta.

Embarazo y lactancia:

Consulte a su médico o farmacéutico antes de utilizar cualquier medicamento.

La seguridad de su uso durante el embarazo y la lactancia no ha sido establecida. Por ello se debe utilizar únicamente cuando sea claramente necesario y cuando los efectos beneficiosos justifiquen los posibles riesgos para el feto o el lactante.

Conducción y uso de máquinas:

No procede por las características de su empleo.

3. Cómo USAR CLORURO DE SODIO FRESENIUS KABI:

Siga exactamente las instrucciones de administración de Cloruro de sodio Fresenius Kabi indicadas por su médico. Consulte a su médico o farmacéutico si tiene dudas.

Las dosis serán adaptadas a la necesidad del paciente según criterio médico en función de la edad, peso, estado clínico, del balance de fluidos, de iones y del equilibrio ácido-base.

Cloruro de Sodio Fresenius Kabi se administra por vía intravenosa, intramuscular o subcutánea.

No es preciso aseptizar la ampolla antes de su apertura. No es necesario ningún elemento cortante para abrir la ampolla. Una vez abierta la ampolla, la boquilla se ajusta perfectamente al cono de la jeringa (cono Luer), con lo cual no es necesario utilizar aguja.

Para abrir separar una ampolla del resto, gire una ampolla sobre sí misma en contra del resto de las ampollas de la tira sin tocar la cabeza y cuello de las ampollas (1).Agitar la ampolla con un único movimiento tal como se muestra en el dibujo para eliminar el líquido que pueda estar en el tapón (2). Para abrir la ampolla, girar el cuerpo y la cabeza de la ampolla en sentidos opuestos hasta que se rompa por el cuello (3). Conectar la jeringa Luer o Luer-lock tal como se muestra en el dibujo (4).

La solución no contiene ningún tipo de conservador ni bactericida, por lo que las ampollas abiertas y no utilizadas deben desecharse inmediatamente.

Si usa más CLORURO DE SODIO FRESENIUS KABI del que debiera:

La sobredosis accidental puede dar lugar a los síntomas descritos del exceso de sodio en sangre y las medidas a tomar serán las correspondientes para la disminución del alto contenido de sodio en sangre.

En caso de sobredosis o ingestión accidental, consulte inmediatamente a su médico o farmacéutico o llame al Servicio de Información Toxicológica, teléfono 91 562 04 20 indicando el medicamento y la cantidad utilizada. Se recomienda llevar el envase y el prospecto al profesional sanitario.

4. Posibles efectos adversos

Como todos los medicamentos CLORURO DE SODIO FRESENIUS KABI puede tener efectos adversos.

Dado que el cloruro de sodio forma parte de la composición normal del medio interno, las reacciones adversas estarían en relación a su concentración ya que es mayor que la del suero de la sangre.

Los efectos adversos descritos lo han sido por administración en exceso o demasiado rápida. Existen dos formas de toxicidad: la primera es el alto contenido de sodio en sangre y la segunda un exceso de sodio y agua que condiciona la acumulación excesiva de líquido que puede afectar al cerebro, pulmones o circulación periférica. Los síntomas del exceso de sodio son náuseas, vómitos, diarrea, calambres abdominales, sed, reducción en la producción de lágrimas y saliva, sudor excesivo, fiebre, taquicardia, aumento de la tensión sanguínea, fallo del riñón, acumulación excesiva de líquido en diferentes zonas del organismo y en los pulmones, parada respiratoria, dolor de cabeza, vértigo, inquietud, debilidad, temblor muscular y rigidez, convulsiones, coma y muerte. Sin embargo, la mayor parte de las hipernatremias descritas han sido como resultado de la utilización de soluciones de cloruro de sodio por vía oral y no por el uso intravenoso de soluciones con concentración de sodio mayor a la del suero de la sangre.

La administración de cantidades importantes de cloro puede dar lugar a una disminución del pH del organismo como consecuencia del descenso de la concentración de bicarbonato.

Si considera que alguno de los efectos adversos que sufre es grave o si aprecia cualquier efecto adverso no mencionado en este prospecto, informe a su médico o farmacéutico.

5. Conservación de CLORURO DE SODIO FRESENIUS KABI

Mantener fuera del alcance y de la vista de los niños.

No requiere condiciones especiales de conservación.

No utilice Cloruro de sodio Fresenius Kabi después de la fecha de caducidad que aparece en el envase después de CAD. La fecha de caducidad es el último día del mes que se indica.

No utilize este medicamento si observa que la solución no es transparente, incolora y libre de partículas.

Una vez abierto el envase, debe usarse el producto inmediatamente.

Los medicamentos no deben tirarse por los desagües ni a la basura. Pregunte a su farmacéutico cómo deshacerse de los envases y de los medicamentos que ya no necesita. De esta forma, ayudará a proteger el medio ambiente.

6. INFORMACIÓN ADICIONAL

Composición de Cloruro de sodio Fresenius Kabi

  • El principio activo es: cloruro de sodio. Cada ml de solución contiene 58,4 mg de cloruro de

sodio (contenido iónico: Na+ = Cl- = 1 mEq/ml).

  • Los demás componentes son: agua para inyección y ácido clorhídrico o hidróxido sódico.

Solución para perfusión para administración por vía intravenosa.

La ampolla de CLORURO DE SODIO FRESENIUS KABI se debe diluir en un volumen mínimo de 50 ml de una solución para infusión.

Úsese únicamente si la solución es transparente y está libre de partículas visibles. Si no se usa todo el contenido, deséchese el resto.

En hiponatremia severa aguda se recomiendan cifras de 1 mmol/l/hora para prevenir el daño neurológico.

Si la hiponatremia es catalogada como crónica, debe asociarse furosemida o un diurético osmótico al suero salino hipertónico.

La corrección de la hiponatremia no debe ser excesivamente rápida para evitar lesiones desmielinizantes. Algunos autores recomiendan no sobrepasar una corrección de 10 mEq/l en las primeras 24 horas y de 21 mEq/l en las primeras 48 horas. En general deberán controlarse los niveles séricos de sodio para detener su reposición en el momento que se alcancen valores normales.
