1. Qué es Bupivacaína B. Braun 7,5 mg/ml y para qué se utiliza

La bupivacaína es un medicamento que pertenece al grupo de los anestésicos locales de tipo amida y está indicado en anestesia local por infiltración, anestesia de conducción, anestesia epidural y espinal, bloqueos diagnósticos y terapéuticos para el tratamiento del dolor y en anestesia epidural y caudal para parto vaginal.

2. Antes de usar Bupivacaína B. Braun 7,5 mg/ml

No use Bupivacaína B. Braun 7,5 mg/ml

Si es alérgico a la bupivacaína, a otros anestésicos locales de tipo amida o a cualquiera de los demás componentes de este medicamento.

No debe usar si tiene problemas graves con la conducción del impulso del corazón, u otros problemas cardíacos, en pacientes con enfermedad nerviosa degenerativa o con defectos graves en la coagulación de la sangre.

El anestésico local no debe inyectarse en regiones infectadas.

Se deben tener en cuenta las contraindicaciones generales y específicas para los distintos métodos de anestesia local y regional.

Advertencias y precauciones

Informe a su médico de cualquier alergia o problema médico que tenga o haya tenido, especialmente:

  • si su hígado no funciona bien ya que su médico deberá ajustar la dosis.
  • si sus riñones no funcionan bien ya que puede aumentar el riesgo de toxicidad.
  • si padece enfermedad por estrechamiento de los vasos sanguíneos, arteriosclerosis (endurecimiento de las arterias) o enfermedad del sistema nervioso debido a diabetes (enfermedad que cursa con exceso de glucosa en sangre).

Uso en niños

Bupivacaína B. Braun 7,5 mg/ml no debería usarse en niños.

Uso en ancianos

Los ancianos requieren dosis menores que los adultos jóvenes o de mediana edad.

Embarazo, lactancia y fertilidad

Si está embarazada o en periodo de lactancia, o cree que podría estar embarazada o tiene intención de quedarse embarazada, consulte a su médico o farmacéutico antes de utilizar este medicamento.

Durante las primeras etapas del embarazo sólo debería ser administrada tras una estricta consideración de las indicaciones.

Cuando se lleve a cabo la anestesia epidural durante las últimas semanas del embarazo, la dosis debe reducirse a aproximadamente un tercio.

Aunque la exposición es menor para la bupivacaína que para otros anestésicos locales, el recién nacido debe ser estrechamente vigilado con respecto a posibles efectos del anestésico.

La bupivacaína pasa a la leche materna, pero en cantidades tan pequeñas que no suponen riesgo para el lactante.

Conducción y uso de máquinas

Bupivacaína B. Braun puede alterar temporalmente su capacidad de movimiento, atención y coordinación. Su médico le indicará si puede conducir o utilizar máquinas.

Bupivacaína B. Braun 7,5mg/ml contiene sodio

Este medicamento contiene 29,9 mg de sodio (componente principal de la sal de mesa/para cocinar) en 10 ml. Esto equivale al 1,5% de la ingesta diaria máxima de sodio recomendada para un adulto.

3. Cómo usar Bupivacaína B. Braun 7,5 mg/ml

Bupivacaína B. Braun 7,5 mg/ml se administra por vía epidural y bloqueo retrobulbar.

Será administrada por personal sanitario únicamente y su médico determinará cual es la dosis más adecuada para usted. Se debe utilizar siempre la dosis más pequeña requerida para producir la anestesia deseada. La dosificación debe ajustarse individualmente de acuerdo con la edad y peso del paciente así como a las particularidades de cada caso.

En pacientes con enfermedades del hígado, riñón, corazón, y en ancianos, es necesario ajustar la dosis.

Si usa más Bupivacaína B. Braun 7,5 mg/ml de la que debe

La intoxicación afecta tanto al sistema nervioso central (SNC) como al sistema cardiovascular (SCV). Pueden distinguirse dos fases: la intoxicación leve a moderadamente severa se caracteriza por estimulación, mientras que la intoxicación severa se caracteriza por sedación y parálisis. Los síntomas son:

Síntomas de la fase de estimulación:

Intoxicación leve

SNC

hormigueo en los labios, parestesia de la lengua, entumecimiento de la boca, tinnitus, sabor metálico, ansiedad, desasosiego, temblores, espasmos musculares, vómitos.

SCV

palpitaciones, hipertensión, taquicardia, taquipnea.

Intoxicación moderadamente grave:

SNC

alteraciones del habla, estupefacción, insomnio, temblores, movimientos coreiformes, convulsiones tónico-clónicas, midriasis, náuseas, vómitos, taquipnea.

SCV

taquicardia, arritmia, palidez, cianosis.

Síntomas de la fase de parálisis:

Intoxicación grave:

SNC

somnolencia, estupor, respiración irregular, parada, respiratoria, pérdida de tono, vómito con aspiración, parálisis de esfínteres, muerte.

SCV

cianosis severa, hipotensión, parada cardíaca, hipo-/asistolia.

La aparición de uno o más síntomas requiere acciones inmediatas.

  1. Cesar la administración del anestésico local.
  2. Asegurar y mantener abierta una vía aérea. Administrar oxígeno que no debe interrumpirse cuando desaparezcan los síntomas sino que debe continuarse varios minutos más.
  3. Si aparecen convulsiones inyectar un barbitúrico de acción ultracorta, como el tiopental (50 - 100 mg) o diazepam (5 - 10 mg), por vía intravenosa en pequeñas dosis repetidas, pero sólo hasta que las convulsiones sean controladas. También se recomienda administrar un relajante muscular de acción corta como la succinilcolina (1 mg/kg de peso corporal), intubar y suministrar respiración artificial con 100% O2.
  4. Controlar inmediatamente la presión sanguínea, el pulso y el diámetro de la pupila.
  5. Si existe hipotensión bajar inmediatamente la cabeza del paciente y administrar un vasoconstrictor que estimule preferentemente el miocardio. Adicionalmente administrar fluidos (p.ej. solución de electrolitos).
  6. Se puede producir una anestesia espinal extensa (total) por inyección intratecal accidental durante la anestesia epidural. Los primeros síntomas son agitación y somnolencia que pueden proseguir a inconsciencia y parada respiratoria. El tratamiento de la anestesia espinal extensa (total) consiste en asegurar y mantener abierta una vía aérea, administrar oxígeno y - si es necesario- instaurar respiración asistida y controlada.

Se asume que las medidas a tomar si se sospecha un paro cardiaco son conocidas. En caso de incidente grave es recomendable consultar a un especialista en anestesiología y cuidados intensivos.

En caso de sobredosis o ingestión accidental, consulte inmediatamente a su médico o farmacéutico o llame al Servicio de Información Toxicológica teléfono: 91 562 04 20, indicando el medicamento y la cantidad utilizada. Lleve este prospecto con usted.

4. Posibles efectos adversos

Al igual que todos los medicamentos, este medicamento puede producir efectos adversos aunque no todas las personas los sufran.

Los posibles efectos secundarios tras la administración de bupivacaína son esencialmente los mismos que los producidos por otros anestésicos locales de tipo amida.

Muy raramente (menos de 1 de 10.000 personas) pueden producirse reacciones alérgicas a anestésicos locales de tipo amida.

Son esperables reacciones sistémicas tóxicas leves a concentraciones plasmáticas superiores a 1,6 - 2 mg/l, mientras que la concentración umbral para las convulsiones es 2 - 4 mg/l. Dichas concentraciones plasmáticas pueden aparecer con dosis excesivas (concentración demasiado elevada o volumen demasiado grande), tras inyección intravascular inadvertida, o tras la administración de dosis estándar a pacientes en situación general deteriorada o con enfermedad del hígado o del riñón.

5. Conservación de Bupivacaína B. Braun 7,5 mg/ml

Mantener fuera de la vista y del alcance de los niños.

No requiere condiciones especiales de conservación.

No utilice Bupivacaína B. Braun después de la fecha de caducidad que aparece en el envase (después de CAD). La fecha de caducidad es el último día del mes que se indica.

El contenido de las ampollas debe ser utilizado inmediatamente tras su apertura. Una vez abierto el envase, desechar la porción no utilizada de la solución.

Los medicamentos no se deben tirar por los desagües ni a la basura. Pregunte a su farmacéutico cómo deshacerse de los envases y de los medicamentos que no necesita. De esta forma ayudará a proteger el medio ambiente.

Se debe utilizar siempre la dosis más pequeña requerida para producir la anestesia deseada:

  • Anestesia epidural:

Las instrucciones de dosificación que se relacionan más adelante se refieren a una administración única a adultos de 70 kg de peso. Es posible la administración repetida o intermitente.

Dosis máxima recomendada para administración única:

Hasta 20 ml de hidrocloruro de bupivacaína 7,5 mg/ml o hasta 2 mg de hidrocloruro de bupivacaína por kg de peso corporal, correspondientes a una dosis máxima de 150 mg.

En pacientes debilitados la dosis debe ser menor de 2 mg/kg de peso corporal.

Para la anestesia epidural la dosis depende de la edad; los niños y los ancianos requieren dosis menores que los adultos jóvenes o de mediana edad. A continuación se indica una guía de dosificación para la región lumbar:

15 años: 1,3 ml/segmento

20 años: 1,5 ml/segmento

40 años: 1,3 ml/segmento

60 años: 1,0 ml/segmento

80 años: 0,7 ml/segmento

Debido al prolongado efecto de la bupivacaína 0,75%, la repetición de la inyección por desaparición de la analgesia sólo se requiere tras al menos 4 horas; esta práctica puede realizarse de forma intermitente o continua. En general, se administran soluciones de menor concentración, p.ej. bupivacaína 0,25%, con esta finalidad.

  • Bloqueo retrobulbar:

De 15 a 30 mg (2 – 4ml).

Para la administración de Bupivacaína B. Braun se debe de tener en cuenta:

  1. Elegir la menor dosificación posible.
  2. Utilizar una aguja de la dimensión apropiada.
  3. Inyectar lentamente con varias aspiraciones en dos planos (rotar la aguja 180º).
  4. No inyectar en regiones infectadas.
  5. Controlar la presión sanguínea.
  6. Tener en cuenta la premedicación. La premedicación debería incluir la administración profiláctica de atropina y - especialmente si es necesario inyectar grandes cantidades de anestésico local - un barbitúrico de acción corta.
  7. Si es necesario, suspender la administración de anticoagulantes antes de la administración del anestésico local.
  8. Observar las contraindicaciones generales y específicas para los diversos métodos de anestesia local o regional.

Manejar en las condiciones habituales de asepsia en la utilización de soluciones inyectables.
