1. Qué es VasoKINOX y para qué se utiliza

VasoKINOX es una mezcla de gases, compuesta por óxido nítrico y nitrógeno, que se emplea para

. el tratamiento de recién nacidos con insuficiencia pulmonarasociada a un aumento de la presión sanguínea en la circulación pulmonar

. el tratamiento de la hipertensión pulmonar aguda(aumento de la presión sanguínea en la circulación pulmonar) que puede darse durante las cirugías cardíacas en adultos, niños y adolescentes, de los 0 a los 17 años.

2. Qué necesita saber antes de empezar a usar VasoKINOX

No use VasoKINOX:

  • si sufre hipersensibilidad al óxido nítrico
  • en neonatos con ciertos problemas cardiacos, como circulación anormal dentro del corazón (derivación derecha a izquierda, o con canal arterial izquierda-derecha “maligno”

Advertencias y precauciones

Consulte a su médico o enfermero antes de empezar a usar VasoKINOX.

  • El tratamiento con VasoKINOX no debe suspenderse de forma brusca.
  • Las muestras de sangre se toman antes del tratamiento y de forma regular durante la administración para controlar ciertos parámetros.
  • Se han notificado casos de retención de líquidos en los pulmones con óxido nítrico en pacientes que padecen una enfermedad causada por una vena bloqueada o estrecha en los pulmones. Si usted (como paciente) o su hijo (como paciente) presenta falta de aliento o dificultad para respirar, póngase en contacto con su médico inmediatamente.

Uso de VasoKINOX con otros medicamentos

Informe a su médico si está utilizando, ha utilizado recientemente o podría tener que utilizar cualquier otro medicamento.

  • VasoKINOX debe administrarse con precaución si los medicamentos mencionados a continuación se toman a la vez:

Lactancia:

Se desconoce si el óxido nítrico/metabolitos se excretan en la leche humana.

Debe tomarse una decisión acerca de si interrumpir la lactancia o interrumpir la terapia con VasoKINOX, teniendo en cuenta las ventajas de la lactancia para el niño y las ventajas de la terapia para la mujer.

Fertilidad:

3. Cómo usar VasoKINOX

Siga exactamente las instrucciones de administración de este medicamento indicadas por su médico. En caso de duda, consulte de nuevo a su médico.

  • VasoKINOX es un gas medicinal administrado únicamente en hospitales por profesionales de la salud.
  • El médico determinará la dosis adecuadade VasoKINOX, así como la duración del tratamiento, dependiendo de su estado clínico y edad.
  • VasoKINOX se administra por inhalación, mediante ventilación mecánicatras la dilución en una mezcla de aire/oxígeno a pacientes en unidades de cuidados intensivos con apoyo respiratorio y también en el quirófano.
  • En contacto con el oxígeno, el óxido nítrico puede convertirse en dióxido de nitrógeno, que irrita los bronquios. Por este motivo, durante su administración, se miden constantemente las concentraciones inhaladas de óxido nítrico y dióxido de nitrógeno.
  • En ciertas circunstancias clínicas, es posible administrar óxido nítrico mediante ventilación con una máscarapor un espacio muy breve de tiempo (de 5 a 10 min).

Si usa más VasoKINOX del que debe:Su médico tomará las medidas oportunas

Si interrumpe el tratamiento con VasoKINOX:

Al finalizar el tratamiento en cuidados intensivos, el médico reducirá gradualmente la dosis de VasoKINOX administrada (fase de retirada), controlando el cambio en su estado.

De hecho, una interrupción brusca del tratamiento tras varias horas de administración puede provocar un deterioro del estado clínico: esto es lo que se denomina “efecto rebote”.

4. Posibles efectos adversos

Al igual que todos los medicamentos, este medicamento puede producir efectos adversos, aunque no todas las personas los sufran.

. Bajo nivel de plaquetas en sangre

. Baja presión arterial, sin aire o colapso pulmonar.

Efectos adversos que pueden aparecer, con poca frecuencia (afectan entre 1 paciente de cada 100 y 1 paciente de cada 1.000) son:

. Aumento de la cantidad de metahemoglobina (forma de hemoglobina que no puede llevar oxígeno) que reduce la cantidad de oxígeno liberada en los tejidos.

Efectos adversos que pueden aparecer pero cuya frecuencia se desconoce (la frecuencia no puede estimarse a partir de los datos disponibles) son:

. Bradicardia (bajo ritmo del corazón) o cantidad demasiado baja de oxígeno en la sangre (desaturación de oxígeno/hipoxemia) debido a una interrupción brusca del tratamiento

5. Conservación de VasoKINOX

  • Mantener este medicamento fuera de la vista y del alcance de los niños.
  • No utilice este medicamento después de la fecha de caducidad que aparece en la etiqueta.
  • Este medicamento no requiere condiciones especiales de temperatura para su almacenamiento.
  • El almacenamiento de las balas de VasoKINOX se efectúa en el hospital.
  • La bala debe protegerse para evitar las roturas y las caídas y debe mantenerse alejada de cualquier material inflamable o combustible y de la humedad.
  • Las balas deben mantenerse en posición vertical y sujetarse bien y deben almacenarse en una zona bien ventilada.
  • Cuando la bala de gas esté vacía, no la deseche. Las balas vacías serán recogidas por el proveedor.

Para evitar cualquier incidente, las siguientes instrucciones deben ser estrictamente respetadas:

. compruebe que el equipo está en buenas condiciones antes de su uso

. asegure firmemente las balas utilizando cadenas o ganchos en la cesta para evitar caídas accidentales

. como el óxido nítrico es incoloro e inodoro, se recomienda el uso de un sistema de detección en todas las habitaciones en las que va a ser utilizado o almacenado

. las agencias de referencia europeas encargadas de la seguridad y salud en el trabajo recomiendan los siguientes límites de exposición para el personal médico durante 8 horas:

  • NO: 2 ppm (2.5 mg/m3)
  • NO2: 0.5 ppm (0.96 mg/m3)

Para que se cumplan las recomendaciones anteriores, se debe implementar un análisis de óxido nítrico y de dióxido de nitrógeno en el atmósfera.

. La instalación de un sistema de canalización de óxido nítrico con una fuente de suministro de balas de gas, red fija y unidades terminales está prohibido.
