1. Qué es Óxido nitroso medicinal Messer y para qué se utiliza

Óxido Nitroso Medicinal Messer es un gas licuado medicinal.

¿Para qué se utiliza?

El tratamiento con óxido nitroso está indicado en los siguientes casos:

  • Coadyuvante de la anestesia general, en asociación con todos los agentes de anestesia administrados por vía intravenosa o por inhalación.
  • Coadyuvante de la analgesia en el quirófano o en la sala de parto.

2. Qué necesita saber antes de iniciar el tratamiento con Óxido nitroso medicinal Messer

No use Óxido nitroso medicinal Messer:

  • Si necesita ventilación con oxígeno puro.
  • Si padece derrames aéricos no drenados, en particular intracraneal o neumotórax, enfisema globuloso.
  • No lo utilice durante más de 24 horas seguidas.

Advertencias y Precauciones

  • Si padece insuficiencia cardiaca, vigile que no tiene una bajada de la tensión arterial.
  • Si va a ser intervenido quirúrgicamente del oído y zonas aéreas adyacentes.

Uso de Óxido nitroso medicinal Messer con otros medicamentos

Informe a su médico si está tomando, o ha tomado recientemente cualquier otro medicamento, incluso los adquiridos sin receta médica.

El óxido nitroso potencia los efectos hipnóticos de los anestésicos intravenosos o por inhalación (tiopental, benzodiazepinas, morfínicos, halogenados), por lo que hay que disminuir la posología de éstos. También debe informar a su médico si está tomando medicamentos que contienen metotrexato (p. ej., para la artritis reumatoide).

Embarazo y lactancia y fertilidad

Embarazo

Durante el embarazo, no se debe superar una concentración de 50% de óxido nitroso en la mezcla inhalada.

Anestesia obstétrica:

Debido al traspaso rápido de la barrera feto-placentaria, se recomienda interrumpir la administración de óxido nitroso si el plazo de tiempo entre la inducción de la anestesia y la extracción del feto sobrepasa los 20 minutos.

Evitar el uso de óxido nitroso en caso de sufrimiento fetal.

Analgesia obstétrica:

La utilización del óxido nitroso en esta indicación sólo puede realizarse si se dispone de un caudalímetro de seguridad que impida la administración de óxido nitroso a una concentración superior al 50%.

Lactancia

Consulte con su médico antes de usar el medicamento.

Conducción y uso de máquinas

Se recomienda no conducir vehículos y no utilizar máquinas durante las 24 horas posteriores a una anestesia con óxido nitroso..

3. Cómo utilizar Óxido nitroso medicinal Messer

El óxido nitroso medicinal se destina a uso exclusivamente hospitalario.

El óxido nitroso solamente debe ser administrado en el quirófano o en la sala de parto.

El médico determinará la dosis necesaria de óxido nitroso y se le administrará mediante un sistema adecuado a sus necesidades que garantizará el suministro de la cantidad correcta de oxígeno

Si Vd. utiliza más Óxido nitroso medicinal Messer del que debiera:

En caso de sobredosificación, existe el riesgo de cianosis y de hipoxia. La administración de óxido nitroso medicinal debe ser interrumpida y el paciente ventilado (aire y oxígeno).

5. Conservación de Óxido nitroso medicinal Messer

Mantener este medicamento fuera de la vista y del alcance de los niños.

No utilizar Óxido nitroso medicinal Messer después de la fecha de caducidad.

Los recipientes criogénicos:

  • Deben ubicarse al aire libre en una zona limpia, sin materiales inflamables, reservada al almacenamiento de los gases de uso medicinal.
  • Está prohibido fumar, engrasar los equipos y hacer fuego en la zona donde se encuentran ubicados los recipientes criogénicos.
  • No se debe estacionar en la zona de los recipientes criogénicos.
  • Se tiene que vigilar el peligro de asfixia.
  • Los recipientes criogénicos deben estar protegidos de los riesgos de choque, en particular los elementos de llenado, de vaciado y de seguridad (válvulas, manómetros), de fuentes de calor o de ignición, de temperaturas superiores a 50ºC y de materiales combustibles.
  • La presión en el interior de las canalizaciones de distribución desde el recipiente criogénico no debe sobrepasar 10 bar.

El óxido nitroso está reservado para uso exclusivo médico.

El óxido nitroso medicinal debe ser utilizado exclusivamente en mezcla con el oxígeno medicinal, la concentración de oxígeno (FiO2) nunca debe ser inferior al 21%.

Para evitar accidentes, se respetarán las siguientes consignas:

  • La proyección de líquido provocaría quemaduras graves de tipo criogénico (muy frío). En caso de quemadura, aclarar abundantemente con agua.
  • Ventilar si es posible el lugar de utilización.
  • Comprobar el buen estado de los materiales antes de la utilización.
  • Efectuar cualquier manipulación sobre recipientes de óxido nitroso llevando guantes limpios adaptados a ese uso y sin nada de grasa y gafas de protección.
  • No tocar nunca las partes frías o con escarcha del material.
  • Manipular el material con las manos limpias y exentas de grasa.
  • Utilizar un racor específico al óxido nitroso.
  • Utilizar flexibles de conexión a las tomas murales provistas de conexiones específicas del óxido nitroso medicinal.
  • No utilizar racor intermedio que permita conectar dos dispositivos que no encajan.
  • Verificar previamente la compatibilidad de los materiales en contacto con el óxido nitroso medicinal, en particular utilizar juntas de conexión al manorreductor previstas para este gas. Comprobar el estado de las juntas.
  • No fumar, no acercarse a una llama y no engrasar.

IMPORTANTE:

  • No introducir nunca este gas en un aparato que pueda ser sospechoso de contener cuerpos combustibles y en particular cuerpos grasos.
  • No limpiar nunca con productos combustibles y en particular cuerpos grasos, los recipientes que contienen este gas, las válvulas, juntas, dispositivos de cierre, así como los circuitos.
  • No aplicar sustancias grasas (vaselina, pomadas, etc.) en el rostro de los pacientes.
  • No utilizar generadores de aerosoles (laca, desodorantes, etc.), de disolvente (alcohol, gasolina, etc.) sobre el material ni en su proximidad.
  • Cuando la temperatura ambiente es baja, el caudal puede disminuir, incluso interrumpirse debido a una presión insuficiente.
  • Evitar utilizar óxido nitroso medicinal a una temperatura inferior a 0 ºC para no provocar una caída de presión en caso de utilización intensiva.
  • No utilizar nunca el óxido nitroso medicinal para ensayos de estanqueidad, para la alimentación de utillaje neumático y para el soplado de tuberías.
  • El valor límite medio de exposición (durante 8 horas) al óxido nitroso se fija en 25 ppm para la exposición del personal.

Efectuar una ventilación sistemática del lugar de utilización, evacuando los gases expirados al exterior y evitando los lugares donde podrían acumularse. Es conveniente, antes de cualquier utilización, asegurarse de la posibilidad de evacuar los gases en caso de accidente o de fuga intempestiva.
