1. Qué es OligoStandard y para qué se utiliza

OligoStandard pertenece al grupo de los electrolitos. Se utiliza como parte de la nutrición intravenosa aportando una fuente de oligoelementos para pacientes adultos.

2. Antes de usar OligoStandard

No use OligoStandard:

  • Si es alérgico (hipersensible) a los principios activos o a cualquiera de los demás componentes de OligoStandard
  • Si padece la enfermedad de Wilson.( enfermedad metabólica hereditaria que produce acumulación de cobre en el hígado)
  • Si padece problemas del hígado como la colestasis pronunciada (disminución de la bilis en el hígado que provoca niveles de bilirrubina superiores a 140 mmol/l y niveles elevados de gammaglutamiltransferasa y fosfatasa alcalina).
  • OligoStandard no debe administrarse a recién nacidos, bebés y niños.

Tenga especial cuidado con OligoStandard:

  • Si la nutrición artificial es prolongada deben controlarle los niveles en sangre de manganeso. Si este elemento se acumula se debe reducir la dosis a administrar o detener la administración.
  • Si padece insuficiencia hepática (alteración del hígado) y excreción biliar reducida (alteración de la vesícula biliar) ya que la eliminación biliar de manganeso, cobre y zinc puede estar afectada, llevando a la acumulación y sobredosis.
  • Si padece insuficiencia renal (alteración del riñón) ya que ésta puede afectar significativamente la excreción renal de cromo y zinc.
  • Si es sometido a nutrición parenteral a medio/largo plazo deben controlarse las necesidades de zinc. Puede ser necesario un suplemento extra de este electrolito.
  • Si padece diabetes mellitusinsulino-dependiente (niveles de glucosa en sangre elevados que requieren la administración de insulina) y deficiencia de cromo, puede requerir una reducción de la cantidad de insulina durante la administración de OligoStandard, ya que la deficiencia de cromo provoca una disminución de la tolerancia a la glucosa, que mejora después del aporte de cromo con OligoStandard.

La diarrea puede provocar un incremento en las pérdidas intestinales de zinc. En este caso deben controlarse las concentraciones séricas de zinc.

3. Cómo usar OligoStandard

Siga exactamente las instrucciones de administración de OligoStandard indicadas por su médico. Consulte a su médico o farmacéutico si tiene dudas.

Su médico indicará al personal sanitario la dosis más adecuada para sus necesidades. Para satisfacer los requerimientos básicos de oligoelementos, se infunden 10 ml de OligoStandard (1 ampolla) diluidos en una solución compatible. Esto corresponde a la dosis diaria normal.

En casos de requerimientos de oligoelementos moderadamente incrementados, pueden ser necesarias dosis superiores (hasta 20 ml, 2 ampollas) controlando los valores séricos de oligoelementos.

En casos de requerimientos significativamente incrementados (tales como quemaduras extensas, pacientes politraumáticos, hipercatabólicos graves y aquellos con pérdida de fluidos intestinales) pueden ser necesarias dosis superiores a la pauta recomendada.

La administración puede ser continuada durante la duración de la nutrición parenteral:

  • Se recomienda precaución en caso de insuficiencia hepática (alteración del hígado), colestasis (disminución de la excreción de bilis en el hígado) y/o insuficiencia renal (alteración del riñón), ya que puede requerirse un ajuste de dosis o suspensión del tratamiento (ver apartado “Tenga especial cuidado con OligoStandard”)

Siempre que sea posible, cualquier deficiencia de un oligoelemento individual debe corregirse mediante suplementos específicos.

Si usted usa más OligoStandard del que debiera:

Su médico suspenderá el tratamiento si los tests del laboratorio confirman una sobredosis.

Al igual que todos los medicamentos, OligoStandard puede producir efectos adversos aunque no todas las personas los sufran.

5. Conservación de OligoStandard

Mantener fuera del alcance y de la vista de los niños.

No se precisan condiciones especiales de conservación.

No utilice OligoStandard si el envase presenta signos visibles de deterioro.

Deséchese la solución si presenta turbidez o sedimentación.

No utilice OligoStandard después de la fecha de caducidad que aparece en el envase. La fecha de caducidad es el último día del mes que se indica.

Los medicamentos no se deben tirar por los desagües ni a la basura. Pregunte a su farmacéutico cómo deshacerse de los envases y de los medicamentos que no necesita. De esta forma ayudará a proteger el medio ambiente.

Período de validez tras la reconstitución:

Desde un punto de vista microbiológico, el producto debe ser utilizado inmediatamente. Si no es utilizado inmediatamente, los tiempos de conservación y las condiciones previas al uso son responsabilidad del usuario y el tiempo no debería ser normalmente superior a 24 horas a 2 - 8 ºC, a menos que la dilución se haya realizado en condiciones asépticas validadas y controladas.

OligoStandard debe administrarse únicamente por vía intravenosa previa dilución en no menos de 250 ml de una solución adecuada para infusión. Las soluciones para infusión que pueden utilizarse son: soluciones de glucosa (ej. glucosa al 5% o al 10%) o soluciones de electrolitos (ej. cloruro sódico al 0,9%, solución de Ringer).

Antes de la adición a otras soluciones para infusión se debe comprobar la compatibilidad. La adición a las soluciones para infusión debe realizarse inmediatamente antes de la administración y en condiciones asépticas.

Esta mezcla puede utilizarse durante 24 horas. Se deben desechar cantidades residuales.

No debe realizarse la administración conjunta con:

  • Soluciones alcalinas
  • Emulsiones lipídicas
  • Soluciones para perfusión intravenosa que contengan vitamina C, ya que los oligoelementos pueden acelerar su degradación
  • Soluciones que contengan fosfato inorgánico (aditivas).
