1. Qué es Imipenem/Cilastatina Steriscience y para qué se utiliza

Imipenem/Cilastatina Steriscience contiene los principios activos imipenem y cilastatina. Pertenece a un grupo de medicamentos denominados antibióticos carbapenémicos. Destruye una gran variedad de bacterias (gérmenes) que causan infecciones en diversas partes del organismo en adultos y niños de un año de edad o más.

Los antibióticos se utilizan para tratar infecciones bacterianas y no sirven para tratar infecciones víricas como la gripe o el catarro.

Es importante que siga las instrucciones relativas a la dosis, el intervalo de administración y la duración del tratamiento indicadas por su médico.

No guarde ni reutilice este medicamento. Si una vez finalizado el tratamiento le sobra antibiótico, devuélvalo a la farmacia para su correcta eliminación. No debe tirar los medicamentos por el desagüe ni a la basura.

2. Qué necesita saber antes de empezar a recibir Imipenem/Cilastatina Steriscience

No debe recibir Imipenem/Cilastatina Steriscience

  • si es alérgico al imipenem, la cilastatina o a alguno de los demás componentes de este medicamento (incluidos en la sección 6)
  • si es alérgico a cualquier otro fármaco antibacteriano carbapenémico
  • si presenta hipersensibilidad grave (p. ej., reacción anafiláctica, reacción cutánea extensa) a cualquier otro tipo de fármaco antibacteriano betalactámico (p. ej., penicilinas o cefalosporinas).

Embarazo y lactancia

Embarazo

Si está embarazada, cree que podría estar embarazada o tiene intención de quedarse embarazada, consulte a su médico antes de recibir este medicamento. No se ha estudiado Imipenem/Cilastatina Steriscience en mujeres embarazadas. Este medicamento no debe usarse durante el embarazo a menos que su médico decida que el posible beneficio justifica el posible riesgo para el bebé en desarrollo.

Lactancia

3. Cómo usar Imipenem/Cilastatina Steriscience

Un médico u otro profesional sanitario prepararán y le administrarán Imipenem/Cilastatina. Su médico decidirá cuánto Imipenem/Cilastatina necesita.

Adultos y adolescentes

La dosis habitual para adultos y adolescentes es de 500 mg de imipenem/500 mg de cilastatina cada 6 horas, o 1.000 mg de imipenem/1.000 mg de cilastatina cada 6 o cada 8 horas. Si tiene problemas renales o pesa menos de 70 kg, su médico puede reducirle la dosis.

Niños

La dosis habitual para niños de un año o más es de 15 mg/15 mg por kg por dosis cada 6 horas. No se recomienda usar este medicamento en niños menores de un año o con problemas renales.

Forma de administración

Tras la reconstitución/dilución, Imipenem/Cilastatina Steriscience se administra por vía intravenosa (en una vena) en el transcurso de 20 a 30 minutos con dosis de ≤500 mg/500 mg o en el transcurso de 40 a 60 minutos con dosis de >500 mg/500 mg. Puede reducirse la velocidad de perfusión en los pacientes que desarrollen náuseas durante la perfusión.

4. Posibles efectos adversos

Al igual que todos los medicamentos, este medicamento puede producir efectos adversos, aunque no todas las personas los sufran.

Efectos adversos graves

Pueden presentarse los siguientes efectos adversos en raras ocasiones:

  • Reacciones alérgicas que comprenden erupción, hinchazón de la cara, los labios, la lengua y/o la garganta (con dificultad para respirar o tragar), y/o presión arterial baja. Si estos efectos adversos se producen mientras recibe o después de recibir Imipenem/Cilastatina Steriscience, debe suspenderse inmediatamente la administración del medicamento y debe informarse a su médico inmediatamente.
  • Reacciones cutáneas graves (síndrome de Stevens-Johnson y eritema multiforme)

  • Náuseas, vómitos, diarrea. Las náuseas y los vómitos resultan más frecuentes en pacientes con un número reducido de glóbulos blancos

  • Pérdida grave de la función hepática debida a una inflamación (hepatitis fulminante)
  • Inflamación del estómago o el intestino (gastroenteritis)
  • Inflamación del intestino con diarrea con sangre (colitis hemorrágica)
  • Hinchazón y enrojecimiento de la lengua, sobrecrecimiento de las proyecciones normales de la lengua que le dan un aspecto velloso, ardor de estómago, dolor de garganta, aumento en la producción de saliva

5. Conservación de Imipenem/Cilastatina Steriscience

Mantener este medicamento fuera de la vista y del alcance de los niños.

No utilice este medicamento después de la fecha de caducidad que aparece en el vial y la caja exterior después de CAD. La fecha de caducidad es el último día del mes que se indica.

No conservar a temperatura superior a 25 °C.

Periodo de validez tras la reconstitución y la dilución:

Las soluciones diluidas deben utilizarse inmediatamente. El intervalo transcurrido entre el comienzo de la reconstitución y el final de la perfusión intravenosa no debe superar las dos horas.

No refrigerar o congelar.

Los medicamentos no se deben tirar por los desagües ni a la basura. Pregunte a su farmacéutico cómo deshacerse de los envases y de los medicamentos que ya no necesita. De esta forma, ayudará a proteger el medio ambiente.

6. Contenido del envase e información adicional

Composición de Imipenem/Cilastatina Steriscience

  • Los principios activos son el imipenem y la cilastatina.

Cada vial contiene imipenem monohidrato equivalente a 500 mg de imipenem y cilastatina sódica equivalente a 500 mg de cilastatina.

  • El otro componente es hidrogenocarbonato de sodio.

Aspecto del producto y contenido del envase

Imipenem/Cilastatina Steriscience es un polvo blanco o casi blanco para solución para perfusión que se presenta en un vial de vidrio de tipo I de 20 ml con tapón de goma de bromobutilo y sello de aluminio de tipo “flip off”.

Cada vial es de un solo uso.

Reconstitución/dilución

El contenido de cada vial debe transferirse a 100 ml de una solución apropiada para la perfusión (ver “Incompatibilidades” más adelante y “Periodo de validez tras la reconstitución y la dilución” en la sección 5 anterior): cloruro sódico 9 mg/ml (0,9%) solución inyectable. En circunstancias excepcionales en las que no puede utilizarse una solución inyectable de cloruro sódico 9 mg/ml (0,9%) por motivos clínicos, puede emplearse glucosa al 5% en su lugar.

Una sugerencia de procedimiento consiste en añadir al vial aproximadamente 10 ml de la solución adecuada para la perfusión.

Agitar bien y transferir la mezcla resultante al recipiente de la solución para perfusión.

PRECAUCIÓN: LA MEZCLA NO ES PARA PERFUSIÓN DIRECTA.

Repetir con otros 10 ml de solución para perfusión con el fin de garantizar la transferencia completa del contenido del vial a la solución para perfusión. La mezcla resultante debe agitarse hasta obtener una solución transparente.

La concentración de la solución reconstituida siguiendo el método anterior es de aproximadamente 5 mg/ml tanto de imipenem como de cilastatina.

La solución solo se debe usar si es transparente y sin partículas.

La solución para perfusión debe examinarse visualmente antes de la administración. El sólido debe disolverse completamente sin dejar partículas visibles. Solo deben utilizarse soluciones transparentes sin partículas.

Las variaciones de color, de incoloro a amarillo, no afectan a la eficacia del medicamento.

Este medicamento no debe mezclarse con otros, excepto con los mencionados en la sección “Reconstitución/dilución” previa.

La eliminación del medicamento no utilizado y de todos los materiales que hayan estado en contacto con él, se realizará de acuerdo con la normativa local.

Incompatibilidades

Este medicamento es químicamente incompatible con el lactato y no debe reconstituirse con diluyentes que contengan lactato. Sin embargo, puede administrarse con un sistema intravenoso a través del cual se perfunda una solución de lactato.
