1. Qué es Fosfato dipotásico 1M Fresenius Kabi y para qué se utiliza

Fosfato dipotásico 1M Fresenius Kabi es un medicamento que pertenece a un grupo llamado soluciones de electrolitos.

Se utiliza para la reposición de fosfatos y potasio en todas aquellas situaciones clínicas en las que es preciso mantener una concentración adecuada de fosfatos y potasio en sangre:

  • Prevención y tratamiento de la hipofosfatemia en pacientes bajo nutrición parenteral y/o en pacientes en unidades de cuidados intensivos.
  • Cetoacidosis diabética como aporte de potasio.

2. Qué necesita saber antes de usar Fosfato dipotásico 1M Fresenius Kabi

  • si es alérgico (hipersensible) al fosfato dipotásico o a cualquiera de los demás componentes de Fosfato dipotásico 1M Fresenius Kabi.
  • si padece de enfermedad grave de riñón.
  • si le han detectado niveles altos de fosfato o potasio en sangre.
  • si padece de episodios de cálculos en el riñón de fosfatos de amonio y magnesio.
  • si padece de infecciones en el riñón, provocadas por microorganismos que hidrolizan la urea.

Advertencias y precauciones

Antes de iniciar el tratamiento con Fosfato dipótasico 1M Fresenius Kabi, su médico debe tener especial precaución y valorar la administración o no de este medicamento:

  • si tiene previamente niveles altos de fosfatos.
  • si tiene previamente niveles altos de potasio.
  • si tiene previamente niveles bajos de calcio.

Se recomienda, por todo lo anterior, monitorización de la función renal, ECG frecuentes y determinaciones periódicas de calcio, fósforo, potasio y sodio séricos.

Debe procurarse no sobrepasar las dosis indicadas pues podría producirse intoxicación por fosfatos e hiperpotasemia (niveles elevados de fosfato y potasio en sangre).

La perfusión intravenosa de fosfatos en cantidades elevadas puede acarrear hipocalcemia (niveles bajos de calcio en sangre).

En administración crónica puede producirse retención de líquidos.

Uso de otros medicamentos

Informe a su médico o farmacéutico si está utilizando o ha utilizado recientemente otros medicamentos, incluso los adquiridos sin receta.

No se recomienda la utilización parenteral de fosfato dipotásico junto a glucósidos digitálicos, en pacientes digitalizados, con bloqueo cardíaco grave o completo, debido a la posible hiperpotasemia (niveles elevados de potasio en sangre).

Si se añade fosfato dipotásico a los pacientes estabilizados en tratamientos con salicilatos, puede producir una elevación de los niveles plasmáticos de éstos hasta concentraciones tóxicas, ya que la acidificación de la orina por los fosfatos hace disminuir la excreción de salicilatos.

La administración concomitante de fosfato dipotásico y antiinflamatorios no esteroideos, captopril, ciclosporina, diuréticos ahorradores de potasio, enalapril, heparina utilizada de forma crónica, lisinopril, leche de bajo contenido en sal, sustitutos de la sal u otros medicamentos que contengan potasio puede aumentar los niveles de potasio en sangre, especialmente en pacientes con disfunción renal.

4. Posibles efectos adversos

Al igual que todos los medicamentos, Fosfato dipotásico 1M Fresenius Kabi puede producir efectos adversos, aunque no todas las personas los sufran.

Los efectos adversos están asociados a una dosis excesiva del medicamento que puede producir niveles elevados de potasio en sangre, niveles bajos de calcio en sangre e intoxicación por fosfatos (tétanos hipocalcémico, con convulsiones, calambres, entumecimiento, hormigueo, temblores y dificultad respiratoria).

No utilice Fosfato dipotásico 1M Fresenius Kabi después de la fecha de caducidad que aparece en el envase después de CAD. La fecha de caducidad es el último día del mes que se indica.

La administración de Fosfato dipotásico 1M Fresenius Kabi es por perfusión intravenosa. El producto requiere ser diluido antes de su uso, como mínimo con una cantidad igual de diluyente.

El producto se puede diluir con soluciones glucosadas al 5% y 10%, ClNa al 0,9% y 0,45% y Lactato sódico 1/6 M. También es compatible durante 24 horas a 22ºC con las soluciones de nutrición parenteral.

El fosfato dipotásico no debe disolverse con soluciones de Ringer, Ringer Lactato y Dextrosa 10% más Cloruro sódico 0,9%.

A la solución de fosfato dipotásico no se le puede añadir dobutamina pues se forman precipitados.

El fosfato dipotásico es compatible con la adición de sulfato magnésico, metoclopramida, verapamil, cefalotina y ceftazidima; en paralelo (perfusión en Y) es compatible con: ciprofloxacino, diltiazem, enalaprilo, esmolol, famotidina, labetalol, vinorelbina y zidovudina.

Usar únicamente si la solución está clara.

La eliminación del medicamento no utilizado y de todos los materiales que se hayan estado en contacto con él, se realizará de acuerdo con la normativa local.
