1. Qué es Corhum solución para cardioplejia y para qué se utiliza

Corhum solución para cardioplejia es una mezcla de electrolitos y aminoácidos.

Corhum solución para cardioplejia se utiliza:

  • en caso de paro cardíaco inducido artificialmente y reversible en cirugía a corazón abierto (cardioplejia en cirugía cardíaca) al provocar un cambio en el equilibrio electrolítico

2. Qué necesita saber antes de empezar a usar Corhum solución para cardioplejia

Corhum solución para cardioplejia solo debe utilizarse de forma intraoperatoria.

No use Corhum solución para cardioplejia

  • si es alérgico a los principios activos o a alguno de los demás componentes de este medicamento (incluidos en la sección 6).

Advertencias y precauciones

Consulte a su médico antes de empezar a usar este medicamento.

Informe a su médico si está embarazada, ya que esto afectará al uso de la solución. Por razones de seguridad, en caso de una cirugía cardíaca de emergencia durante el embarazo, su médico aspirará la solución de perfusión de la aurícula y el ventrículo derechos de su corazón tras finalizar la operación.

Este medicamento no está destinado a la administración directa en un vaso sanguíneo, sino únicamente a la perfusión del corazón detenido. Por lo tanto, este medicamento no debe utilizarse para perfusión sistémica.

Según el tipo de cirugía, el método utilizado, la duración de la intervención y el tamaño del paciente, pueden entrar hasta 3 litros de solución de perfusión en todo su sistema circulatorio. Esto equivale al 17% de la ingesta diaria máxima recomendada de sodio para un adulto.

Corhum solución para cardioplejia contiene 352 mg de potasio por 1000 ml. A tener en cuenta por pacientes con función renal reducida o pacientes con dieta controlada en potasio.

3. Cómo usar Corhum solución para cardioplejia

El plan de tratamiento y la dosis deben ser determinados por un médico especialista. Este medicamento solo debe ser utilizado por profesionales sanitarios, es decir, un médico o un enfermero.

La ingesta de grandes cantidades de Este medicamento en la circulación sanguínea puede provocar una sobrecarga de volumen y desequilibrios electrolíticos (niveles sanguíneos demasiado bajos de calcio o sodio y niveles sanguíneos demasiado altos de magnesio o potasio). En tal caso, su médico realizará controles de laboratorio periódicos.

5. Conservación de Corhum solución para cardioplejia

Conservar en nevera (entre 2ºC y 8ºC) y conservar las bolsas en el embalaje exterior para protegerlas de la luz.

No congelar. Usar inmediatamente después de la apertura.

Mantener este medicamento fuera de la vista y del alcance de los niños.

Este medicamento no debe utilizarse después de la fecha de caducidad que aparece en el envase después de «CAD». La fecha de caducidad es el último día del mes que se indica.

La eliminación del medicamento no utilizado y de todos los materiales que hayan estado en contacto con él se realizará de acuerdo con la normativa local. De esta forma, ayudará a proteger el medio ambiente.

Advertencias y precauciones especiales de empleo

La inactivación completa del miocardio lo hace susceptible al estiramiento excesivo. Por lo tanto, se necesita un drenaje adecuado del ventrículo. No se deben superar los volúmenes y presiones de perfusión recomendados.

Se deben tomar precauciones especiales cuando se trate de corazones de lactantes y niños.

Una perfusión inadecuada con solución cardiopléjica no suficientemente enfriada (>20°C y >15 minutos) puede provocar la llamada «paradoja del calcio» y conducir a la destrucción de las células miocárdicas una vez implantado el órgano.

Este riesgo no se aplica a las soluciones con un contenido de sodio <20 mmol/l, si el contenido de calcio es >10 μmol/l y la solución está fría, es decir, <15°C y limitada en el tiempo, es decir perfundida continuamente durante no más de 20 minutos. En cambio, la inmersión de un corazón entero en Corhum solución para cardioplejia fría no conlleva ningún riesgo de paradoja del calcio aunque dure horas, por ejemplo, durante el intervalo de tiempo entre la extracción del órgano de un donante y el trasplante a un receptor. Por lo tanto, cuando se usa de forma correcta, la paradoja del calcio es prácticamente imposible, incluso en condiciones experimentales extremas.

Sobredosis

La ingesta de grandes volúmenes de Corhum solución para cardioplejia en la circulación sistémica puede provocar un volumen circulante excesivo con el consiguiente desequilibrio electrolítico (hipocalcemia, hiponatremia, hipermagnesemia, hiperpotasemia). Por lo tanto, se recomienda un control adecuado de los electrolitos séricos.
