1. Qué es Bupivacaína Hiperbárica B. Braun 5 mg/ml Mini-Plasco y para qué se utiliza

La bupivacaína es un anestésico local de larga duración. Produce un bloqueo acusado de las fibras nerviosas.

Bupivacaína Hiperbárica B. Braun se utiliza para anestesia espinal indicada en intervenciones de las extremidades inferiores y el perineo, intervenciones en el abdomen inferior, parto vaginal normal y cesárea y cirugía reconstructiva de las extremidades inferiores.

2. Qué necesita saber antes empezar a usar Bupivacaína Hiperbárica B. Braun 5 mg/ml Mini-Plasco

No use Bupivacaína Hiperbárica B. Braun 5 mg/ml Mini-Plasco

  • Si es alérgico (hipersensible) a los principios activos o a cualquiera de los demás componentes de Bupivacaína Hiperbárica B. Braun.
  • Si usted tiene:
  • enfermedades cardiovasculares, sobre todo bloqueo cardíaco o shock,
  • dolor de espalda crónico,
  • enfermedad del sistema nervioso central preexistente atribuible a infección, tumores u otras causas,
  • trastornos de la coagulación sanguínea, tratamiento con anticoagulantes,
  • dolor de cabeza, especialmente si existe historial de migrañas,
  • presión arterial baja o elevada (hipertensión o hipotensión arterial),
  • alteraciones de la sensibilidad como consecuencia de lesiones del sistema nervioso central de tipo persistente (sensación continua de hormigueo, entorpecimiento, etc.),
  • deformaciones o características espinales que puedan interferir con la administración y/o eficacia del anestésico,
  • líquido cefalorraquídeo (liquido que baña el cerebro y la médula espinal) hemorrágico o
  • antecedentes de hipertermia maligna (elevación de la temperatura corporal y contracciones musculares intensas).

Advertencias y precauciones

Utilizar con precaución si el paciente padece alguna enfermedad (por ejemplo insuficiencia cardíaca) o está recibiendo a la vez alguna terapia que disminuya el flujo de sangre en el hígado.

Es importante que informe a su médico si utiliza alguno de los siguientes medicamentos, debido a que éstos pueden interaccionar:

  • Heparina (medicamento utilizado para prevenir la formación de coágulos en sangre).
  • Antirreumáticos esteroideos (AINES) (previenen la formación de coágulos) orales (acenocumarol, warfarina).
  • Sustitutivos plasmáticos (se utilizan para recuperar el volumen sanguíneo después de una hemorragia).

Embarazo, lactancia y fertilidad

Si está embarazada o en periodo de lactancia, o cree que podría estar embarazada o tiene intención de quedarse embarazada, consulte a su médico o farmacéutico antes de utilizar este medicamento.

Durante las primeras etapas del embarazo, sólo debería ser administrada después de una estricta consideración de las indicaciones.

Cuando se lleve a cabo la anestesia en el parto normal o por cesárea deben administrarse las dosis indicadas para este tipo de pacientes.

No se conoce con exactitud si la bupivacaína pasa a leche materna, aunque no se han descrito riesgos para el lactante se recomienda usar con precaución en madres lactantes.

Conducción y uso de máquinas

Bupivacaína Hiperbárica B. Braun puede alterar temporalmente su capacidad de movimiento, atención y coordinación. Su médico le indicará si puede conducir o utilizar máquinas.

Bupivacaína Hiperbárica B. Braun 5 mg/ml Mini-Plasco contiene sodio

Este medicamento contiene menos de 23 mg de sodio (1 mmol) por 1 mililitro; esto es, esencialmente “exento de sodio”.

3. Cómo usar Bupivacaína Hiperbárica B. Braun 5 mg/ml Mini-Plasco

Este medicamento siempre lo administrará personal sanitario mediante inyección dentro del líquido cefalorraquídeo a través del espacio lumbar. Vía intratecal.

La dosificación del hidrocloruro de bupivacaína varía en función del tipo de intervención quirúrgica y su duración y la respuesta individual de cada paciente.

Las dosis más utilizadas deben reducirse cuando se trata de niños, pacientes geriátricos o debilitados, y pacientes con enfermedades hepáticas y/o cardíacas.

Se utilizará siempre la dosis más baja requerida para producir el efecto deseado.

Si usa más Bupivacaína Hiperbárica B. Braun 5 mg/ml Mini-Plasco de la que debe

Al igual que con los otros anestésicos locales, debido a una excesiva dosificación, una rápida absorción o una inyección accidental en vasos sanguíneos pueden presentarse síntomas de intoxicación que pueden manifestarse:

  • Sobre el sistema nervioso central:

Caracterizados por anestesia de la lengua, pérdida de conciencia ligera, desvanecimiento, visión borrosa, dolor de cabeza, temblores seguidos por somnolencia, convulsiones e inconsciencia.

  • Sobre el aparato respiratorio:

Aceleración de la respiración, seguida de dificultad/ parada respiratoria.

  • Sobre el aparato circulatorio:

Vasodilatación periférica, hipotensión, depresión del miocardio, conducción atrio-ventricular retardada y arritmias, incluso paro cardíaco.

Tratamiento de emergencia, antídotos

Si se producen efectos tóxicos, la primera medida es cesar la administración del anestésico local, y recurrir al tratamiento de los síntomas presentes incluyendo, si es necesario, respiración asistida.

En caso de sobredosis o ingestión accidental, consulte inmediatamente a su médico o farmacéutico o llame al Servicio de Información Toxicológica teléfono: 91.562 04 20, indicando el medicamento y la cantidad utilizada. Lleve este prospecto con usted.

4. Posibles efectos adversos

Al igual que todos los medicamentos, este medicamento puede producir efectos adversos, aunque no todas las personas los sufran.

En la mayor parte de los casos, están relacionados con dosis altas, por una inyección accidental en vasos sanguíneos o por reacciones alérgicas o tolerancia disminuida por parte del paciente.

A las dosis recomendadas y con los procedimientos anestésicos adecuados no se han observado efectos secundarios importantes.

A diferencia de otros anestésicos con bupivacaína no se han descrito casos de disminución de hemoglobina en sangre y las reacciones alérgicas son muy raras.

Los efectos secundarios por sobredosis o dificultades en la técnica anestésica pueden originar dolor de espalda o dolor de cabeza, incontinencia fecal y/o urinaria, hipotensión, alteración de la sensibilidad y parálisis de las extremidades inferiores, problemas respiratorios y ritmo cardíaco lento.

5. Conservación de Bupivacaína Hiperbárica B. Braun 5 mg/ml Mini-Plasco

No require condiciones especiales de conservación.

Deséchese la solución si presenta turbidez o sedimentación.

No utilizar si el envase presenta signos visibles de deterioro.

Mantener fuera de la vista y del alcance de los niños.

No utilice Bupivacaína Hiperbárica B. Braun después de la fecha de caducidad que aparece en el envase después de CAD. La fecha de caducidad es el último día del mes que se indica.

Los medicamentos no se deben tirar por los desagües ni a la basura. Pregunte a su farmacéutico cómo deshacerse de los medicamentos que ya no necesita. De esta forma ayudará a proteger el medio ambiente.

Bupivacaína hidrocloruro 5,0 mg

Los demás componentes son: glucosa monohidrato, hidróxido de sodio y agua para preparaciones inyectables.

Bupivacaína Hiperbárica 5 mg/ml Mini-Plasco es una solución inyectable que se presenta en ampollas de polietileno (Mini-Plasco®) de 4 ml.

Forma de administración

Es fundamental tener en cuenta:

  • Elegir la dosis tan baja como sea posible.
  • Utilizar una aguja de la dimensión apropiada.
  • Inyectar lentamente con varias aspiraciones en dos planos (rotar la aguja 180º).
  • No inyectar en regiones infectadas.
  • Controlar la presión sanguínea si las dosis son elevadas.
  • Administrar tranquilizantes a dosis moderadas antes de la medicación anestésica.
  • Tener disponible el equipo necesario para la reanimación para tratar cualquier complicación que pueda suceder.
  • Practicar una inyección test del 5%-10% de la dosis.
  • Mantener contacto verbal con el paciente y controlar los signos cardiovasculares. Si se producen síntomas de toxicidad cesar la administración.

Utilizar con precaución si el paciente padece alguna enfermedad (insuficiencia cardíaca) o está recibiendo a la vez alguna terapia que disminuya el flujo de sangre en el hígado.

Tener prudencia con las dosis en casos de hipoxia, aumento de la tasa hemática de potasio o acidosis, ya que estas situaciones aumentan el riesgo de toxicidad cardíaca de bupivacaína, así como en niños menores de 12 años, en ancianos y en sujetos con insuficiencia renal o hepática.

Dosificación:

En términos generales, puede decirse que es suficiente una dosis de 10 mg por acto quirúrgico, pero determinados pacientes y procedimientos especiales pueden requerir más o menos dosis.

Tratamiento de emergencia, antídotos

Si se producen efectos tóxicos, la primera medida es cesar la administración del anestésico local.

El subsiguiente tratamiento consiste en parar las convulsiones y asegurar la respiración adecuada con oxígeno, si es necesario por respiración asistida. Si se producen convulsiones pueden tratarse con 5 - 10 mg de diazepam. Si existe hipotensión debe administrarse intravenosamente un vasopresor, por ejemplo 15 - 30 mg de efedrina.
