1. Qué es Oponaf y para qué se utiliza

Oponaf pertenece al grupo de medicamentos denominados laxantes osmóticos orales.

Oponaf está indicado:

  • En el tratamiento del estreñimiento habitual crónico.
  • Como parte del tratamiento de algunos síntomas de la encefalopatía portosistémica, que es una consecuencia de una enfermedad grave del hígado.

2. Qué necesita saber antes de empezar a tomar Oponaf

El estreñimiento crónico debe ser tratado, como primera medida, mediante dieta rica en fibra, ingestión suficiente de líquidos o ejercicio físico. Si no se produce una mejora con estas medidas, su médico puede recomendarle el uso de Oponaf.

Especialmente en niños, debe tenerse en cuenta que el tratamiento prolongado con laxantes puede entorpecer el funcionamiento normal del reflejo de defecación, por lo que se recomienda tratar el estreñimiento con medidas de higiene y dietética adecuadas.

Debe evitarse el uso de laxantes durante períodos prolongados.

No tome Oponaf

  • si es alérgico (hipersensible) al lactitol;
  • si tiene obstrucción intestinal o hay sospecha de lesiones orgánicas en el tracto digestivo;
  • si existe dolor abdominal no justificado o sangrado rectal;
  • si se está siguiendo una dieta sin galactosa;
  • en caso de estasis fecal (retención prolongada), que deberá ser tratada previamente por otros medios;
  • si padece galactosemia;
  • si está usted amamantando a un lactante con intolerancia hereditaria autosómica recesiva a la fructosa;
  • si padece un desequilibrio preexistente de electrolitos en suero.

Advertencias y precauciones

  • en pacientes de edad avanzada, debilitados o con deshidratación o hipopotasemia (bajos niveles de potasio en sangre), su médico deberá controlar regularmente el nivel de electrolitos en sangre, especialmente en los tratamientos prolongados;
  • si se le tiene que practicar cualquier examen interno del intestino, debe consultar a su médico;
  • en casos de ileostomía o colostomía (abertura desde el íleon o el colon al exterior) no está recomendada la administración de Oponaf;
  • en caso de estreñimiento particularmente resistente, debe consultar a su médico;
  • en el caso de que se produzcan náuseas, es recomendable tomar Oponaf durante las comidas;
  • en bebés, solo se debe usar Oponaf tras la recomendación del médico.

Toma de Oponaf con otros medicamentos

Informe a su médico o farmacéutico si está utilizando o ha utilizado recientemente cualquier otro medicamento, incluso los adquiridos sin receta médica.

Ciertos medicamentos pueden interaccionar con Oponaf; en estos casos puede ser necesario cambiar la dosis o interrumpir el tratamiento con alguno de ellos.

Es importante que informe a su médico si está tomando o ha tomado recientemente alguno de los siguientes medicamentos:

  • Antiácidos o neomicina (antibiótico) no deben administrarse conjuntamente con Oponaf en los tratamientos de cirrosis con encefalopatía hepática, ya que estos medicamentos pueden interferir en su mecanismo de acción.
  • Diuréticos tiazídicos (medicamentos utilizados para aumentar la eliminación de orina).
  • Corticosteroides (medicamentos antiinflamatorios y antialérgicos).
  • Carbenoxolona (protector de la mucosa gástrica y antiinflamatorio).
  • Anfotericina B (medicamento para el tratamiento de las infecciones producidas por hongos).
  • Mesalazina (medicamento antiinflamatorio).

Embarazo y lactancia

Consulte a su médico o farmacéutico antes de utilizar cualquier medicamento.

No se utilizará Oponaf durante el primer trimestre del embarazo a no ser que su médico le recomiende lo contrario.

Aunque no se ha estudiado el paso del lactitol a la leche materna, el uso de Oponaf se considera seguro para madres en periodo de lactancia.

Conducción y uso de máquinas

Oponaf no ejerce ningún efecto sobre la capacidad para conducir vehículos o utilizar maquinaria.

Oponaf contiene lactitol

Si su médico le ha indicado que padece una intolerancia a ciertos azúcares, consulte con él antes de tomar este medicamento.

El lactitol tiene un valor calórico de 2,1 kcal/g.

Oponaf no afecta a los niveles de glucosa o insulina en sangre, por lo que es apto para pacientes diabéticos.

El uso de Oponaf no afecta a los dientes.

3. Cómo tomar Oponaf

Siga exactamente las instrucciones de administración de Oponaf indicadas por su médico. En caso de duda, consulte de nuevo a su médico o farmacéutico.

Estreñimiento

El tratamiento con Oponaf debe ser adaptado de forma individual, con el fin de conseguir una deposición diaria.

Oponaf se administra por vía oral en una dosis única diaria, por la mañana o por la noche, preferiblemente mezclado con los alimentos o la bebida y tomando 1-2 vasos de líquido durante la comida. El paciente deberá escoger entre tomar el medicamento por la mañana o por la noche, según su respuesta, ya que el efecto laxante ocurre mayoritariamente unas horas después de la toma.

La primera respuesta laxante puede ser que no aparezca hasta el segundo o tercer día de tratamiento.

Uso en niños: La dosis diaria inicial es de 0,25 g/kg de peso corporal por día; una pauta general puede ser la siguiente:

  • de 1 a 6 años: 2,5 a 5 g de polvo/día (¼-½ sobre)
  • de 6 a 12 años: 5 a 10 g de polvo/día (½-1 sobre)
  • de 12 a 16 años: 10 a 20 g de polvo/día (1-2 sobres)

La toma de Oponaf por los niños debe ser supervisada por un adulto y deben seguirse estrictamente las instrucciones del médico.

Adultos (incluyendo pacientes de edad avanzada): Oponaf, se administra por vía oral aunque, en pacientes hospitalizados puede administrarse también por sonda nasogástrica o enema de retención vía catéter de balón rectal, según las indicaciones del médico.

La dosis inicial diaria debe ser de 2 sobres (20 g de lactitol) en una sola toma. Después de algunos días, una dosis diaria de 1 sobre (10 g) puede ser suficiente para muchos pacientes.

En caso de no resultar eficaz la dosis inicial, puede llegar a administrarse una dosis máxima de 3 sobres (30 g/día).

Encefalopatía hepática:

El tratamiento con Oponaf debe adaptarse a cada individuo. Es importante seguir las instrucciones del médico. La dosis inicial para el tratamiento de adultos es de 0,5 a 0,7 g/kg de peso corporal por día. La dosis diaria debe dividirse en 3 tomas, junto con las comidas, bebiendo 1-2 vasos de líquido durante la comida.

La dosis indicada puede variar hasta conseguir dos deposiciones blandas por día, dependiendo de la gravedad de la enfermedad y de la respuesta al tratamiento.

Si estima que la acción de Oponaf es demasiado fuerte o débil, comuníqueselo a su médico o farmacéutico.

Su médico le indicará la duración de su tratamiento con Oponaf. No suspenda el tratamiento antes del tiempo indicado, ya que un uso irregular o la interrupción prematura del tratamiento conlleva el riesgo de recaída.

Si toma más Oponaf del que debiera

Si ha tomado más Oponaf de lo que debe, consulte inmediatamente a su médico o a su farmacéutico.

El signo de sobredosificación con Oponaf es la diarrea, que puede desaparecer disminuyendo la dosis. Si la diarrea persiste, consulte a su médico.

Si olvidó tomar Oponaf

Tome la próxima dosis a la hora habitual.

No tome una dosis doble para compensar las dosis olvidadas.

4. Posibles efectos adversos

Al igual que todos los medicamentos, Oponaf puede producir efectos adversos, aunque no todas las personas los sufran:

Al inicio del tratamiento pueden producirse molestias abdominales y flatulencia; a veces se puede presentar dolor o espasmos de estómago. Estos efectos tienden a disminuir o desaparecer al cabo de unos días de la toma regular de Emportal

Debido a las diferencias entre individuos, algunos pacientes pueden experimentar diarrea a las dosis recomendadas. Este problema se resuelve con la reducción de la dosis.

5. Conservación de Oponaf

Mantener este medicamento fuera de la vista y del alcance de los niños.

No conservar a temperatura superior a 30ºC. Conservar protegido de la humedad.

La solución preparada debe conservarse preferentemente entre 2ºC y 8ºC (en nevera) y debe consumirse en un máximo de 2 días.

No utilice Oponaf después de la fecha de caducidad que aparece en el envase después de la abreviatura “CAD”. La fecha de caducidad es el último día del mes que se indica.

Los medicamentos no se deben tirar por los desagües ni a la basura. Deposite los envases y los medicamentos que no necesita en el Punto SIGRE de la farmacia. En caso de duda pregunte a su farmacéutico cómo deshacerse de los envases y de los medicamentos que no necesita. De esta forma, ayudará a proteger el medio ambiente.

El principio activo es lactitol monohidrato. Un sobre contiene 10 g de lactitol monohidrato.

Oponaf no contiene ningún excipiente.
