1. Qué es Mucofluid y para qué se utiliza

Mucofluid es un medicamento mucolítico que se une a unas proteínas de los fragmentos del moco y lo solubiliza, favoreciendo su eliminación.

Vía nebulización se utiliza en:

  • mucoviscidosis,
  • patologías obstructivas del árbol respiratorio por tapón mucoso.

Vía instilación endotraqueal se utiliza:

  • para facilitar la aspiración y drenaje de mucosidades y exudados retenidos en el tracto respiratorio durante la anestesia o cuidados intensivos,

en broncoscopia, para asegurar una mejor visión de las áreas a examinar.

2. Antes de usar Mucofluid

No use Mucofluid

  • Si es alérgico (hipersensible) a mesna, a otros compuestos con grupo tiol o sulfonamidas o a cualquiera de los demás componentes de Mucofluid.
  • Si padece asma sin obstrucción mucosa. O en caso de estatus asmático.
  • Si padece insuficiencia renal grave.
  • Si presenta intolerancia a los aerosoles.
  • Si no puede toser o expectorar adecuadamente.
  • En niños menores de 2 años.

Tenga especial cuidado con Mucofluid

  • Mucofluid es exclusivamente para uso mediante nebulizador o instilación endotraqueopulmonar y debe administrarse únicamente bajo vigilancia médica.
  • No se recomienda su uso en niños menores de 12 años.
  • En los enfermos asmáticos el producto se administrará solamente si hay retención de mucosidades y exudados bronquiales y siempre en presencia de un médico o en un centro sanitario.
  • Se puede utilizar cualquier tipo de aparato, a condición de que el circuito a través del cual tiene que pasar el producto sea de materia plástica, de vidrio o de acero inoxidable. Mucofluid no debe estar en contacto directo con caucho o metal sin proteger.
  • Se puede formar un depósito blanquecino en los aparatos o circuitos, que puede eliminarse enjuagando con agua.
  • Idealmente la solución debe ser administrada a temperatura ambiente.
  • Por ser un producto fácilmente oxidable, es preferible abrir la ampolla en el momento de usar su contenido. La solución puede ser usada hasta 24 horas después de su apertura, conservada por debajo de 25ºC.
  • La inyección accidental intravenosa o absorción oral no pone en peligro al paciente. Sin embargo, la inyección accidental intramuscular puede causar necrosis tisular local (muerte del tejido por falta de riego sanguíneo).

Uso de otros medicamentos

Informe a su médico o farmacéutico si está utilizando o ha utilizado recientemente otros medicamentos, incluso los adquiridos sin receta.

Mucofluid puede usarse simultáneamente mediante aplicación local con otros medicamentos como antibióticos (para tratar infecciones), broncodilatadores (para tratar trastornos respiratorios de tipo asmático), corticoides (para tratar procesos inflamatorios) y anestésicos cuando estos son administrados vía intravenosa.

Se recomienda no mezclar en la misma solución Mucofluid con aminoglucósidos (estreptomicina, canamicina, neomicina, gentamicina), ya que éstos se inactivan. Sin embargo, no existe inconveniente para la utilización de todos estos antibióticos mediante vía sistémica y Mucofluid mediante aplicación local.

Por razones de incompatibilidad fisicoquímica se recomienda no mezclar Mucofluid en la misma solución con:

  • cierto tipo de antibióticos (oxitetraciclina, carbenicilina y eritromicina)
  • aminofilina (medicamento para tratar el asma)
  • lipiodol (medio de contraste radiológico)
  • cisplatino (medicamento para tratar ciertos tumores)
  • gas nitrógeno

Si le realizan un análisis de orina pueden aparecer resultados falsos positivos en la determinación de determinados componentes (sulfitos o cetonas), debido a la presencia en la orina de algunas sustancias (disulfuros o grupos tioles libres) después de la administración de mesna.

Embarazo y lactancia

Consulte a su médico o farmacéutico antes de utilizar cualquier medicamento.

No existen datos suficientes sobre la utilización de mesna en mujeres embarazadas.

Mucofluid no debe utilizarse durante el embarazo excepto si fuese claramente necesario.

Mucofluid debe evitarse durante la lactancia o la lactancia debe interrumpirse, mientras recibe el tratamiento con Mucofluid.

Conducción y uso de máquinas

3. Cómo usar Mucofluid

Siga exactamente las instrucciones de administración de Mucofluid indicadas por su médico. Consulte a su médico si tiene dudas.

Mucofluid puede administrarse:

Mediante nebulización:La solución se puede utilizar pura, o puede diluirse a partes iguales con agua destilada o suero fisiológico. La solución puede administrarse preferiblemente con una pieza nasal, con una máscara o bajo una cámara.

La dosis terapéutica usual es 3 ml (600 mg) a 6 ml (1200 mg) del producto sin diluir al día, repartidos en 3 a 4 veces al día.

La dosis máxima es 24 ml (4,8 g) de producto puro al día.

Mediante instilación endotraqueal:La solución se debe diluir a partes iguales, con agua destilada o suero fisiológico. La solución puede administrarse via tubo endotraqueal o cánula de traqueotomía.

La dosis terapéutica usual es 1 ml ó 2 ml de solución diluída al 10% cada hora.

La dosis máxima es 24 ml (4,8 g) al día.

El tratamiento debe reducirse al periodo más corto posible y, en caso de no existir mejoría suficientemente rápida, se debe reevaluar la situación clínica con un posible ajuste del tratamiento. En mucoviscidosis, Mucofluid se administrará en función de la sintomatología.

Si usa más Mucofluid del que debiera:

No se han notificado casos de sobredosis.

Tras la utilización de grandes volúmenes de solución y la licuefacción repentina de la mucosidad, puede producirse un encharcamiento pulmonar parcial (acumulación parcial de líquido en los pulmones), causando trastornos transitorios en la ventilación. Se debe vigilar estrictamente al paciente y realizar tratamiento sintomático.

Si olvidó usar Mucofluid:

No use una dosis doble para compensar las dosis olvidadas.

Si interrumpe el tratamiento con Mucofluid:

4. Posibles efectos adversos

Al igual que todos los medicamentos, Mucofluid puede tener efectos adversos, aunque no todas las personas los sufran.

Los efectos adversos descritos con Mucofluid son:

-Dolor de pecho.

-Hipersensibilidad (alergia).

-Tos.

-Broncoespasmo (contracción de los bronquios, acusando su estrechamiento y dificultad al respirar).

  • Disgeusia (alteración en el sentido del gusto (mal sabor, sabor amargo,…)).

-Edema angioneurótico (ronchas o habones por hinchazón bajo la piel).

-Rash eritematoso (erupción cutánea rojiza).

-Urticaria (picor).

Existe la posibilidad de broncoespasmo, particularmente en pacientes asmáticos sensibles. Puede aparecer, especialmente en niños, vómitos y molestias digestivas ocasionadas por la excesiva fluidificación e ingestión de las mucosidades.

Algunas veces puede ocurrir una sensación de quemazón retroesternal (zona central del pecho, por detrás del esternón) con la solución a una concentración de 200 mg/ml (en este caso la solución se debe diluir a 100 mg/ml).

Si considera que alguno de los efectos adversos que sufre es grave o si aprecia cualquier efecto adverso no mencionado en este prospecto, informe a su médico o farmacéutico.

5. Conservación de Mucofluid

Mantener fuera del alcance y de la vista de los niños.

No utilice este medicamento después de la fecha de caducidad que aparece en el envase después de CAD. La fecha de caducidad es el último día del mes que se indica.

Este medicamento no requiere condiciones especiales de conservación.

Por ser un producto fácilmente oxidable, es preferible abrir la ampolla en el momento de usar su contenido. La solución puede ser usada hasta 24 horas después de su apertura, conservada por debajo de 25ºC.

Los medicamentos no se deben tirar por los desagües ni a la basura. Deposite los envases y los medicamentos que no necesita en el Punto SIGRE de la farmacia. En caso de duda pregunte a su farmacéutico cómo deshacerse de los envases y de los medicamentos que no necesita. De esta forma, ayudará a proteger el medio ambiente.

6. Información adicional

Composición de Mucofluid

  • El principio activo es mesna. Cada ml de Mucofluid contiene 200 mg de mesna. Cada ampolla de 3 ml contiene 600 mg de mesna.
  • Los demás componentes son: edetato de disodio, hidróxido de sodio (para ajuste del pH) y agua para preparaciones inyectables.
