Pityriasis rosea

Pityriasis rosea is a benign rash that appears suddenly, follows a very characteristic pattern and clears on its own within 6–8 weeks.

Pityriasis rosea is a benign rash that appears suddenly, follows a very characteristic pattern and clears on its own within 6–8 weeks. It is thought to relate to reactivation of human herpesviruses 6 and 7, which almost everyone carries from childhood; it is not an allergy, not a fungus and barely contagious at all. What matters here is recognising it and excluding two things that resemble it: syphilis and a drug reaction.

How it develops

  • herald patch: first a single oval patch appears, pink or brown, 2–10 cm across, with a collarette of fine scale turned inwards at the edge. It is usually on the trunk;
  • one to two weeks later the generalised rash erupts: smaller oval patches with the same appearance;
  • they follow the skin's cleavage lines, forming a pattern on the back that resembles a Christmas tree;
  • sites: trunk, neck, upper arms and thighs; the face, palms and soles are usually spared;
  • mild to moderate itching in most cases, sometimes none;
  • occasionally there is malaise, a sore throat or a low fever a few days beforehand;
  • it clears within 6–8 weeks, sometimes up to 12, leaving pale or dark marks that even out over months;
  • on brown and black skin the lesions may be more papular than patchy, with more involvement of neck and face and more marked and longer-lasting pigment change.

What it can be confused with

This is the part that changes decisions:

  • secondary syphilis: the rash is similar, but it involves the palms and soles, there is no herald patch and lymph nodes are usually enlarged. If there is any doubt, or if there has been unprotected sex, serology is done. This is a diagnosis not to miss;
  • pityriasis versicolor: patches with irregular edges, no oval shape and no collarette, identified by skin scraping;
  • ringworm of the body: few lesions, an actively scaling border, more itching;
  • guttate psoriasis: red papules with thicker scale, often after a throat infection;
  • discoid eczema;
  • drug reaction: some medicines produce very similar rashes, which is why your doctor will review your medication.

When to see a doctor

  • to confirm the diagnosis, particularly the first time;
  • there are lesions on the palms or soles;
  • you have had unprotected sex in the preceding months;
  • the rash appeared after starting a new medicine;
  • itching is severe and disturbing your sleep;
  • there is fever, joint pain or significant malaise;
  • the rash has not cleared within three months;
  • you are pregnant: in the first trimester it is worth discussing with your doctor;
  • there are blisters or the lesions are painful.

How it is diagnosed

By the appearance and distribution, which are highly characteristic. A scraping is done to exclude fungus, and syphilis serology if there is any doubt or any risk factor. Biopsy is exceptional. The doctor will ask about medicines taken in recent weeks.

Treatment

No treatment is needed for it to clear: it resolves by itself. What gets treated is the itch.

  • emollients and soothing lotions;
  • mild topical corticosteroids in a short course if it itches;
  • oral antihistamines, particularly at night;
  • short lukewarm showers, avoiding hot water, which worsens the itch;
  • gentle cleansers instead of soap;
  • loose cotton clothing;
  • brief, sensible sun exposure: it speeds resolution for some people, but without burning;
  • in very extensive cases of recent onset the doctor may consider aciclovir;
  • antifungals and antibiotics are of no use: this is neither a fungus nor a bacterium.

Common questions

Is it contagious? Practically not. Cases in several members of a household are rare, and there is no need to isolate or stay off work or school.

Will it come back? It is uncommon: most people have it only once in their lives.

Will it leave marks? There is no scarring. Paler or darker marks do remain and even out over weeks or months, lasting longer on darker skin.

Can I swim in a pool or the sea? Yes, rinsing afterwards and applying a moisturiser.

Can it be speeded up? Not reliably. Treatment relieves the itch, but the natural course is what it is.

Online consultation

In an online consultation the doctor reviews photographs and the arrangement of the lesions, checks whether the palms and soles are involved — the finding that makes syphilis testing necessary — reviews your medicines and advises on treating the itch and what to expect week by week.

Dr. Andrei Popov is a General Practitioner with specialized training in chronic pain. He provides online consultations for adult patients in Spain and other European countries.

Dr. Popov understands that seeking help with weight-related concerns or pain that has persisted for months can be difficult. Many patients come to him after trying different approaches without feeling truly heard. During the consultation, the doctor creates a comfortable and supportive, non-judgmental environment where the patient’s medical history can be discussed in detail, questions can be answered, and the next steps can be decided together.

Weight Management and Metabolic Health

Dr. Popov can help assess overweight, obesity, and related conditions such as hypertension, diabetes, or metabolic disorders. During the consultation, the patient’s medical history, test results, current medications, and previous treatments will be reviewed.

If you would like to learn about possible medication options or have already started treatment, the doctor can assess your situation, review the effectiveness and tolerability of the therapy, and clearly explain its benefits, potential risks, and alternatives.

Not every patient requires the same treatment. A consultation does not guarantee a prescription — it provides an objective medical assessment and a plan tailored to your individual situation.

Pain and General Medicine

You can also consult Dr. Popov regarding:

* chronic pain, migraines, neuralgia, and fibromyalgia;

* back, neck, lower back, or joint pain;

* hypertension, diabetes, and follow-up of chronic conditions;

* review of test results, medical reports, and current treatment;

* preventive medicine and a second medical opinion.

How the Consultation Works

The video consultation lasts up to 30 minutes. The doctor will listen carefully, review the information you provide, and explain the next steps in clear and understandable language.

If additional tests, an in-person consultation, or urgent medical care are required, the doctor will inform you directly.

Dr. Popov’s goal is to make sure the patient feels heard and leaves the consultation with a clear understanding of their condition and the next steps they can take.
